低温等离子消融术治疗儿童阻塞性睡眠呼吸暂停低通气综合征的疗效观察

目的:探讨低温等离子消融术治疗儿童阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的疗效。方法:入选的58例OSAHS患者为我院耳鼻喉科2008年5月至2009年5月入院治疗患者,全部患者均应用美国Arthrocare公司的ENT CoblatorlI等离子系统治疗仪,连接EVac 70 T&A刀头,切除扁桃体和(或)消融腺样体,并于术前及术后6个月行多导睡眠仪监测治疗效果。结果:58例患者治愈15例,显效24例,有效12例,无效7例,总有效率87.9%。所有患者术后未出现呼吸、吞咽困难等并发症,多数患者诉鼻阻塞和轻微疼痛,术后2周患者的临床症状明显减轻或消失。术后AHI、最低血氧饱和度(LSpO2)、打鼾次数与术前比较,差异有统计学意义(P<0.05)。结论:低温等离子消融术治疗儿童阻塞性睡眠呼吸暂停低通气综合征疗效满意,并发症少,值得广泛推广和应用。 Objective: To investigate the therapeutic effect of low temperature plasma ablation on obstructive sleep apnea-hypopnea syndrome (OSAHS) in children. Methods: Fifty-eight patients with OSAHS admitted to our hospital from May 2008 to May 2009 were enrolled. All patients were treated with the ENT Coblator II plasma system from Arthur’s company in the United States. The EVac 70 T & A was removed and resected Tonsil and (or) ablation of adenoid, and preoperative and postoperative six months polysomnography to monitor the treatment effect. Results: Fifty-eight patients were cured in 15 cases, effective in 24 cases, effective in 12 cases, ineffective in 7 cases, the total effective rate was 87.9%. All patients did not appear after breathing, dysphagia and other complications, the majority of patients complained of nasal obstruction and minor pain, 2 weeks after the patient’s clinical symptoms significantly reduced or disappeared. The postoperative AHI, the lowest oxygen saturation (LSpO2), the number of snoring and preoperative comparison, the difference was statistically significant (P <0.05). CONCLUSION: Low temperature plasma ablation is a satisfactory and effective method for the treatment of children with obstructive sleep apnea-hypopnea syndrome. It is worthy of widespread application.
