目的:通过对老年2型糖尿病患者血清脂联素、瘦素水平检测及二者之间的比值分析,探讨老年2型糖尿病胰岛素抵抗发生的机理,为临床诊断治疗提供依据。方法:用酶联免疫分析和放射免疫分析对249例老年2型糖尿病和90例正常对照者及216例老年2型糖尿病经一年治疗前后脂联素、瘦素水平进行检测。结果:老年2型糖尿病患者脂联素水平明显低于正常对照组(P<0.01);瘦素水平2型糖尿病均高于正常对照组(P<0.01);216例老年2型糖尿病治疗前后比较,脂联素、瘦素及二者比值差异有显著(P<0.01),脂联素与瘦素比较呈负相关。结论:老年2型糖尿病胰岛素抵抗的发生与脂联素、瘦素密切相关,二者的比值变化对糖尿病分型具有重要意义。
OBJECTIVE: To explore the mechanism of insulin resistance in senile type 2 diabetes mellitus (T2DM) by detecting the serum levels of adiponectin and leptin in senile patients with type 2 diabetes mellitus and the ratio between them, and to provide the basis for clinical diagnosis and treatment. Methods: The levels of adiponectin and leptin in 249 elderly type 2 diabetic patients, 90 normal controls and 216 elderly type 2 diabetic patients after one year treatment were detected by ELISA and radioimmunoassay. Results: The level of adiponectin in elderly type 2 diabetic patients was significantly lower than that of normal controls (P <0.01); the level of type 2 diabetes mellitus was higher in leptin than in normal controls (P <0.01); 216 elderly type 2 diabetic patients before and after treatment , Adiponectin, leptin and the ratio of the two were significantly different (P <0.01), adiponectin and leptin was negatively correlated. Conclusion: The incidence of insulin resistance in type 2 diabetes mellitus is closely related to adiponectin and leptin, and the ratio of them is of great significance for the typing of diabetes mellitus.
