目的探讨颅底硬膜外表皮样囊肿恶变的临床特征及预后。方法报告颅底硬膜外表皮样囊肿恶变1例,结合相关文献分析其临床、影像学及病理特点,探讨其诊断、治疗及预后。结果颅底硬膜外表皮样囊肿恶变临床比较少见。影像学对表皮样囊肿典型病例诊断不难,恶变病例较困难,常需术后病理。外科治疗为首选。结论表皮样囊肿恶变机制目前不清,治疗应在手术切除基础上给予放射和化学治疗,预后欠佳。
Objective To investigate the clinical characteristics and prognosis of skull base epidural dermoid cyst malignancy. Methods One case of malignant epidural epidermoid cyst of skull base was reported. The clinical, radiographic and pathological features of the skull base epidural surface cyst were analyzed. The diagnosis, treatment and prognosis were analyzed. Results Skull epidural epidermoid cyst malignant clinical relatively rare. Radiographic imaging of epidermoid cysts is not difficult to diagnose typical cases, malignant cases more difficult, often requiring postoperative pathology. Surgical treatment is preferred. Conclusions The mechanism of malignant transformation of epidermoid cyst is unclear. The treatment should be given radiotherapy and chemotherapy on the basis of surgical excision, and the prognosis is poor.
