报告14例杂种猪上腹部器官簇移植的术中、术后生理学指标变化。平均冷缺血时间为154±26分钟,平均转流时间64±13分钟。受体平均手术时间222±31分钟。受体术中平均失血1600±210ml,平均输血935±65ml。手术结束后20分钟平均体温33.14±1.83℃,下降3±1.53℃。手术前后血pH、HCO-3及动脉血二氧化碳分压(PaCO2)指标改变显示大部分受体(10/14)术后出现代谢性酸中毒,8例有呼吸性代偿。所测11例受体手术前后动脉血氧分压(PaO2)、血氧饱和度无明显变化,血氧含量有所下降。3例移植物单纯用Colins液灌注而不做灌洗者出现高血钾症。血流动力学以无肝前手术开始后20分钟值为对照,测得转流早期、开放后早期的平均动脉压、中心静脉压及心输出量有较大变化。认为在术中减少出血、维持体温正常、维持酸碱平衡以及血流动力学平稳,是保证手术顺利进行的关键
Report 14 cases of hybrid pigs abdominal organ cluster transplantation intraoperative and postoperative changes in physiological indicators. The average cold ischemia time was 154 ± 26 minutes, the average flow time of 64 ± 13 minutes. Recipients average operative time 222 ± 31 minutes. Receiver surgery average blood loss 1600 ± 210ml, the average blood transfusion 935 ± 65ml. 20 minutes after surgery the average body temperature 33.14 ± 1.83 ℃, down 3 ± 1.53 ℃. Changes of blood pH, HCO-3 and PaCO2 before and after surgery showed that metabolic acidosis occurred in most of the recipients (10/14), and respiratory compensation occurred in 8 patients. The measured partial pressure of arterial oxygen (PaO2) before and after the operation of the 11 recipients showed no significant changes in the oxygen saturation and the blood oxygen content decreased. Three cases of transplant with Colins fluid perfusion without laxity appear hyperkalemia. Hemodynamics in 20 minutes after the start of hepatic pre-operative value as a control, measured in the early flow, the early open after the average arterial pressure, central venous pressure and cardiac output have a greater change. It is considered as the key to ensure the smooth operation of the operation in terms of reducing bleeding during operation, maintaining normal body temperature, maintaining acid-base balance and steady hemodynamics
