目的探讨血清抗突变型瓜氨酸波形蛋白(MCV)抗体、抗环瓜氨酸肽(CCP)抗体、类风湿因子(RF)、超敏C-反应蛋白(hs-CRP)和全血红细胞沉降率(血沉,ESR)在类风湿关节炎(RA)早期诊断中的临床价值。方法对98例RA患者、88例疾病对照组和56例健康对照组分别采用酶联免疫吸附试验(ELISA)方法检测血清抗MCV抗体、抗CCP抗体浓度;采用免疫散射比浊法测定血清hs-CRP、RF的浓度;用ALIFAX TEST1全自动血沉仪检测ESR,并进行分析比较。结果 RA患者抗MCV抗体、抗CCP抗体、RF、hs-CRP和ESR均明显高于疾病对照组和健康对照组(P<0.01)。抗MCV抗体具有较高的敏感性和特异性(90.8%,93.8%)。五项指标联合检测的特异性最高,达到99.3%,其敏感性为61.2%。结论抗MCV抗体在RA的诊断中具有较高的敏感性和特异性,更有利于RA的早期诊断。五项指标联合检测的特异性最高,可进一步提高RA早期诊断的准确性。
Objective To investigate the effects of serum anti-mutant MCV antibody, anti-cyclic citrullinated peptide (CCP) antibody, rheumatoid factor (RF), hs-CRP and whole blood erythrocyte sedimentation rate Rate (ESR) in the early diagnosis of rheumatoid arthritis (RA) in the clinical value. Methods Serum levels of anti-MCV antibodies and anti-CCP antibodies were measured in 98 patients with RA, 88 patients with disease control and 56 healthy controls by enzyme linked immunosorbent assay (ELISA). The serum hs- CRP, RF concentrations; ESR was detected by ALIFAX TEST1 automatic ESR, and analyzed and compared. Results The anti-MCV antibody, anti-CCP antibody, RF, hs-CRP and ESR in RA patients were significantly higher than those in the control group and healthy controls (P <0.01). Anti-MCV antibodies have higher sensitivity and specificity (90.8%, 93.8%). Joint detection of the five indicators of the highest specificity, reaching 99.3%, the sensitivity was 61.2%. Conclusion Anti-MCV antibodies have high sensitivity and specificity in the diagnosis of RA, and more conducive to the early diagnosis of RA. Joint detection of the five indicators of the highest specificity, can further improve the accuracy of early diagnosis of RA.
