目的:了解无创呼吸机对慢性阻塞性肺疾病合并呼吸衰竭的临床疗效,总结临床经验,为疾病的治疗提供有效方法。方法:选择我院2010年12月至2013年12月期间就诊的慢性阻塞性肺疾病合并呼吸衰竭的患者138例作为研究对象,按照数字表随机抽样的方法将患者分为实验组与对照组,每组患者各69例,对照组患者采用常规治疗,实验组患者在对照组的基础上实施无创呼吸机治疗,对比两组患者的临床疗效进行分析对比。结果:两组患者经过治疗后心率、呼吸频率等较治疗前均有明显好转,实验组的患者的好转程度大于对照组,即:对照组治疗后的心率为(18.32±1.65)次/m i n次/m i n,呼吸频率为(24.68±2.34)次/min;二氧化碳分压为(54.98±4.96)mmHg,动脉血氧分压为(64.76±4.26)mmHg;血氧饱和为(90.21±1.43)%;平均住院时间为(13.5±5.7)天,实验组治疗后的心率为(80.34±3.68)次/min;呼吸频率为(87.49±6.37)次/min;(46.29±3.21)mmHg;动脉血氧分压为(78.54±3.21)mmHg;两组数据结果比较差异具有统计学意义(P<0.05);血氧饱和度为(96.97±2.98)%;平均住院时间为(10.3±4.1)天。其差异具有统计学意义,P<0.05。结论:在慢性阻塞性肺疾病合并呼吸衰竭的治疗过程中采用无创呼吸机具有良好的临床效果,能有效减短患者的住院时间,减轻患者的痛苦,是值得临床上进行推广使用的有效方法。“,”Objective: To evaluate the efficacy of noninvasive ventilator in chronic obstructive pulmonary disease with respiratory failure. Methods: A total of 138 patients with chronic obstructive pulmonary disease (COPD) and respiratory failure (RF) were included in our hospital between December 2010 and December 2010. According to the random method, the patients were equal y divided the experimental group and control group. Conventional treatments were used on 69 cases in control group, while the patients in experimental group received additional noninvasive ventilator based on the conventional method. The clinical curative effects of two groups were statistical y analyzed. Results: There were significantly improved both in control group and experiment group on the terms of heart rate (80.34±3.68bpm vs 87.49±6.37bpm, P<0.05) and breathing rate (18.32±1.65bpm vs 24.68±2.34bpm, P<0.05) after received therapy. The improved degree of CO2 partial pressure in experiment group (54.98±4.96mmHg) was significant higher than that in control group (46.29±3.21mmHg) (P<0.05). The arterial blood oxygen partial pressure and blood oxygen saturation in experiment group was 78.54±3.21mmHg and 96.97±2.98%, respectively, showing a significant increase in contrast to control group (64.76±4.26mmHg; 90.21±1.43%) (P<0.05), but the hospitalization in experiment group was shorter than that in control group (10.3±4.1days vs 13.5±5.7days, P<0.05). Conclusion: Patients with COPD and RF can benefit from the noninvasive ventilator.
