目的:探讨社区人群中体质指数(body mass index,BMI)与脉搏波传导速度(PWV)的关系。方法:入选南京市鼓楼区40岁以上社区自然人群3 381人,填写调查问卷,收集疾病史、药物治疗史和生活习惯等基本信息,测量身高、体重、腰围、血压,计算BMI,行口服葡萄糖耐量试验(OGTT),测定血脂及PWV。结果:多元逐步线性回归分析显示收缩压(SBP)是PWV的独立危险因素(β=0.478,P<0.001)。协方差分析结果显示校正混杂因素后,PWV和BMI间存在弱的负性相关。代谢健康肥胖者PWV明显低于代谢异常的正常体重者(P<0.01)。结论:BMI不是PWV的独立影响因素,BMI和PWV间的关系很大程度上可以通过BMI和血压的强阳性关系进行解释。
Objective: To investigate the relationship between body mass index (BMI) and pulse wave velocity (PWV) in community population. METHODS: A total of 3 381 natural population aged 40 years or older in Drum Tower District of Nanjing were enrolled in the questionnaire. Basic information such as disease history, medication history and lifestyle were collected. Height, weight, waist circumference and blood pressure were measured. BMI was calculated. Tolerance test (OGTT), determination of blood lipids and PWV. Results: Multiple stepwise linear regression analysis showed that systolic blood pressure (SBP) was an independent risk factor for PWV (β = 0.478, P <0.001). Analysis of covariance showed that there was a weak negative correlation between PWV and BMI after adjusting for confounding factors. Metabolic health obese PWV was significantly lower than those with normal metabolic abnormalities (P <0.01). CONCLUSION: BMI is not an independent predictor of PWV, and the relationship between BMI and PWV can largely be explained by the strong positive relationship between BMI and BP.
