目的探讨2型糖尿病患者血浆同型半胱氨酸(Hcy)检测的临床意义。方法选取本院2014年5月至2015年10月收治的54例2型糖尿病患者,根据检测患者的24h尿白蛋白排泄率的水平不同而将54例患者分为三个小组:18例患者的尿白蛋白排泄率UAE<30mg/24h即为单纯2型糖尿病患者,则设为2型组;18例UAE30-300mg/24h即为微量组;18例UAE>300mg/24h则设为大量组;另选取同期到院进行检测的18例正常者,设为对照组。测定四组的血浆Hcy水平,并进行对比分析,探讨血浆同型半胱氨酸检测临床意义。结果微量组、大量组的血浆Hcy检测结果均明显高于对照组(P<0.05);微量组、大量组又明显较2型组的高(P<0.05);而2型组与对照组差异不明显;并且经本研究统计发现,高Hcy的2型糖尿病患者的糖尿病肾病临床发病率要明显高于其他人群中的患者(P<0.05);另外,各组间的其他各项生化检测数据的对比差异均有统计学意义(P<0.05)。结论 2型糖尿病患者的血浆同型半胱氨酸水平与24h尿白蛋白为正相关,故检测2型糖尿病患者的血浆Hcy为临床诊断病情及提供治疗具有积极的指导意义。
Objective To investigate the clinical significance of detecting plasma homocysteine (Hcy) in type 2 diabetic patients. Methods Fifty-four patients with type 2 diabetes mellitus admitted from May 2014 to October 2015 in our hospital were divided into three groups according to the level of 24h urinary albumin excretion rate: 18 patients Urinary albumin excretion rate UAE <30mg / 24h is simple type 2 diabetes, then set to type 2 group; 18 cases of UAE30-300mg / 24h is the trace group; 18 cases of UAE> 300mg / 24h is set to a large number of groups; In addition, 18 normal subjects were selected as the control group in the same period. Plasma Hcy levels in four groups were determined and compared to explore the clinical significance of plasma homocysteine. Results The results of plasma Hcy in micro-group and large-scale group were significantly higher than those in the control group (P <0.05), while those in the micro-group and the large-volume group were significantly higher than those in the model group (P <0.05) Obviously, the incidence of diabetic nephropathy in type 2 diabetes mellitus with high Hcy was significantly higher than those in other groups (P <0.05). In addition, the other biochemical data The differences were statistically significant (P <0.05). Conclusions Plasma homocysteine levels in type 2 diabetic patients are positively correlated with 24 h urinary albumin. Therefore, detecting plasma Hcy levels in patients with type 2 diabetes has a positive guiding significance in clinical diagnosis and treatment.
