目的 :探讨血清甲胎蛋白 (AFP)、癌胚抗原 (CEA)、糖抗原 19- 9(CA19- 9)的测定对消化道恶性肿瘤的诊断价值。方法 :采用放射免疫法对 132例经内镜及影像学检查确诊的消化道肿瘤患者 ,12 0例消化系良性病患者 ,50例健康人员的AFP、CEA、CA19- 9进行测定。结果 :消化道恶性肿瘤组血清AFP、CEA、CA19- 9阳性诊断率明显高于健康组、良性疾病组 (P <0 .0 1) ;AFP仍是肝癌诊断的最敏感的标志物 ;CEA特异性差 ,但在消化系肿瘤中有较高阳性率 ;CA19- 9对胰腺癌的诊断有特别意义。结论 :AFP、CEA及CA19- 9三者联合检测可提高肿瘤的检出率 ,有时虽不能作出何种肿瘤的诊断 ,但可提示存在隐匿性病灶。
Objective: To investigate the diagnostic value of serum alpha-fetoprotein (AFP), carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9) in the diagnosis of gastrointestinal malignancies. Methods: AFP, CEA and CA19-9 were measured by radioimmunoassay in 132 patients with gastrointestinal cancer diagnosed by endoscopy and imaging examination, 120 patients with digestive tract benign disease and 50 healthy persons. Results: The positive rate of serum AFP, CEA and CA19-9 in gastrointestinal malignant tumor group was significantly higher than that in healthy group and benign disease group (P <0.01). AFP was still the most sensitive marker for liver cancer diagnosis. CEA specific Poor sex, but in digestive tumors have a higher positive rate; CA19-9 for the diagnosis of pancreatic cancer have special significance. Conclusion: The combined detection of AFP, CEA and CA19-9 can improve the detection rate of tumors. Sometimes, although the diagnosis of tumors can not be made, it may indicate the existence of occult lesions.
