目的 探讨CD4 0L CD4 0与儿童单纯性肾病综合征 (INS)免疫紊乱的关系。方法 采用流式细胞术检测 2 0例INS患儿外周血淋巴细胞CD4 0L CD4 0的表达及INS患儿血浆对 10名健康儿童CD4 0L CD4 0表达的影响。同时以 2 0名健康儿童作为对照组。结果 (1)INS组CD4 0L的表达与对照组比较差异无显著性(P >0 0 5 ) ,而CD4 0的表达较对照组增高 (P <0 0 1)。经植物血凝素 (PHA)预刺激后INS组CD4 0L、CD4 0的表达均较前增高 (P <0 0 1,P <0 0 5 ) ,但最终CD4 0L仍低于经PHA预刺激后对照组的表达 (P <0 0 1)。经PHA预刺激后的对照组在INS血浆的孵育下CD4 0L表达明显下降 (P <0 0 1) ,CD4 0的表达不受影响。 (2 )经PHA预刺激后INS组CD4 0L的低表达与血清IgG降低、IgM升高有显著相关 (r =0 5 9,- 0 5 6 ;P <0 0 1,0 0 5 ) ,与IgA、2 4h尿蛋白程度无明显相关 (r =- 0 18,0 0 4 ;均P >0 0 5 )。CD4 0表达与血清IgG、IgM、IgA、2 4h尿蛋白水平均无明显相关 (r =0 2 6 ,- 0 32 ,0 12 ,0 0 4 ;均P >0 0 5 )。结论 INS患儿CD4 0L表达降低与疾病状态时的免疫紊乱有密切关系。INS患儿血浆能降低CD4 0L的表达 ,可能与某些细胞因子有关。
Objective To investigate the relationship between CD4 0L CD4 0 and immune disorders in children with simple nephrotic syndrome (INS). Methods The expression of CD4 0L CD4 0 in peripheral blood lymphocytes and the expression of CD4 0L CD4 0 in 10 healthy children were detected by flow cytometry. At the same time to 20 healthy children as a control group. Results (1) The expression of CD4 0L in INS group was not significantly different from that in control group (P> 0.05), while the expression of CD4 0 in control group was significantly higher than that in control group (P <0.01). After pretreatment with phytohemagglutinin (PHA), the expression of CD4 (superscript +) T and CD4 (superscript +) in INS group were higher than those before (P <0.01, P <0.05) Control group expression (P <0.01). The control group after pre-stimulation with PHA had a significant decrease in the expression of CD4OL (P <0.01), while the expression of CD4 0 was not affected. (2) The low expression of CD4 0L in INS group after pre-stimulation with PHA was significantly associated with the decrease of serum IgG and the increase of IgM (r = 0 59- 0 56; P 0 01 0 05) IgA, 24h urine protein level was not significantly correlated (r = - 0 18,0 0 4; all P> 0 0 5). There was no significant correlation between the expression of CD4 0 and serum IgG, IgM, IgA, 24 h urinary protein levels (r = 0 2 6, - 0 32, 0 12, 0 0 4; all P 0 05). Conclusion The decrease of CD4 0L in children with INS is closely related to the immune disorder in the disease state. INS plasma can reduce the expression of CD4 0L may be related to certain cytokines.
