目的:观察清热益气通络方对糖尿病肾病(DN)大鼠肾小球podocalyxin、desmin表达影响。方法:将SD大鼠随机分为正常组(N)和糖尿病肾病造模组(采用单侧肾切除腹腔注射链脲佐菌素的方法建立大鼠糖尿病肾病模型)。将造模成功的大鼠再分为模型对照组(M)、中药组(Q)和厄贝沙坦组(I)。干预治疗12w后处死大鼠,检测、计算各组大鼠24 h微量白蛋白(umAlb)、24 h尿蛋白量(uPro)、内生肌酐清除率(Ccr)。免疫组织化学染色的方法观察肾小球podocalyxin、desmin蛋白的表达。光镜及电镜观察肾小球结构。结果:①Q组大鼠24 h umAlb、SCr均低于M组(P<0.05),24 h uPro显著低于M组(P<0.01)。②与N组相比Q组大鼠肾小球Podocalyxin表达显著降低(P<0.01),但明显高于M组(P<0.05);Q组大鼠肾小球Desmin表达高于N组(P<0.01),低于M组(P<0.05)。结论:清热益气通络中药能减弱足细胞上皮间充质转分化(EMT),从而降低DN大鼠蛋白尿,改善肾功能。
Objective: To observe the effect of “Qingre Yiqi Tongluo Decoction” on glomerulus podocalyxin and desmin expression in diabetic nephropathy (DN) rats. Methods: SD rats were randomly divided into normal group (N) and diabetic nephropathy model group (diabetic nephropathy model was established by intraperitoneal injection of streptozotocin). The successful rats were divided into model control group (M), Chinese medicine group (Q) and irbesartan group (I). The rats were sacrificed 12 weeks after intervention, and the 24 h microalbuminuria, 24 h urinary protein (uPro) and endogenous creatinine clearance (Ccr) of rats in each group were measured and calculated. Immunohistochemical staining was used to observe the expression of podocalyxin and desmin in glomeruli. Light microscopy and electron microscopy glomerular structure. Results: (1) 24 h umAlb and SCr in Q group were significantly lower than those in M group (P <0.05), 24 h uPro was significantly lower than M group (P <0.01). ② Compared with N group, the expression of Podocalyxin in glomerulus in Q group was significantly decreased (P <0.01), but significantly higher than that in M group (P <0.05). The expression of Desmin in Glomerular Q group was higher than that in N group <0.01), lower than M group (P <0.05). Conclusion: “Qingre Yiqi Tongluo Chinese Herb” can reduce the epithelial mesenchymal transition (EMT) of podocytes, thus reducing proteinuria and improving renal function in DN rats.
