目的对罕见B(A)亚型进行血清学和基因定型,探讨该类血型患者的输血策略。方法采用试管凝集法鉴定血型,采用序列特异性引物-聚合酶链式反应法进行ABO基因分型,采用凝聚胺法进行交叉配血试验。结果试管凝集法检测3例样本定型为A弱B强,其血清中存在抗-A抗体;3例样本经血型抗原基因突变数据库比对,基因型均为B(A)亚型,其中1种样本为B(A)04/O01单体型,2种样本为B(A)04/O02单体型,3例样本均在第640位碱基发生了A>G的突变;3例样本与A、B、O、AB型供血者进行交叉配血,结果主侧或次侧不合,与O型、B型红细胞交叉配血结果相合,与AB型血浆交叉配血结果相合。结论罕见B(A)亚型的鉴定及血清学特性分析对后续患者的输血策略具有较大影响。
Objective To investigate the serological and genotyping of rare B (A) subtypes and to explore the transfusion strategies of this type of blood group. Methods The blood type was identified by tube agglutination test. The ABO genotyping was performed by sequence specific primer - polymerase chain reaction (PCR), and the crossmatch test was performed by polybrene method. Results The results of tube agglutination assay showed that the three samples were scored as A weak and strong B, and anti-A antibody was present in the serum. The three samples were compared with the genotypes of B (A) The sample was B (A) 04 / O01 haplotype and the two samples were B (A) 04 / O02 haplotype. A> G mutation occurred at the 640th base in all three samples. Three samples A, B, O, AB type blood donor cross-matching blood, the results of the main side or subsidence, and O, B type red blood cells cross match the results of the blood, and AB type plasma cross matching blood results. Conclusion The identification of rare B (A) subtypes and the analysis of serological characteristics have a great influence on the transfusion strategy of follow-up patients.
