目的:研究术前腔内放疗联合新辅助化疗对Ⅱa和Ⅱb期宫颈癌组织中细胞凋亡的影响。方法:选择在我院接受术前辅助治疗的Ⅱa和Ⅱb期局部晚期宫颈癌患者进行回顾性分析,根据术前辅助治疗方案不同分为辅助化疗组、腔内放疗组、联合治疗组。活检时(治疗前)和手术切除时(治疗后)分别留取宫颈癌组织,抽提RNA后采用荧光定量PCR试剂盒测定促凋亡基因、凋亡抑制基因、侵袭基因的表达量。结果:治疗后,三组宫颈癌病灶内SARI、Rab26、FHIT、eIF4E3的表达量均显著高于治疗前,血清中CA125、TSGF、SCC-Ag的含量以及MRP8、MRP14、p38MAPK、ERK1/2、c-myc的表达量均显著低于治疗前且联合治疗组宫颈癌病灶内SARI、Rab26、FHIT、eIF4E3的表达量均显著高于辅助化疗组、腔内放疗组,血清中CA125、TSGF、SCC-Ag的含量以及MRP8、MRP14、p38MAPK、ERK1/2、c-myc的表达量均显著低于辅助化疗组、腔内放疗组。结论:术前腔内放疗联合新辅助化疗能够促进Ⅱa和Ⅱb期宫颈癌组织中的表达凋亡。
Objective: To investigate the effect of preoperative intracavitary combined with neoadjuvant chemotherapy on apoptosis in stage Ⅱa and Ⅱb cervical cancer. Methods: The patients with stage Ⅱa and Ⅱb advanced cervical cancer undergoing preoperative adjuvant therapy in our hospital were retrospectively analyzed. According to the preoperative adjuvant therapy, the patients were divided into adjuvant chemotherapy group, endovascular radiotherapy group and combined therapy group. Cervical cancer tissues were collected during biopsy (before treatment) and after surgical resection (after treatment). RNA was extracted and the expression levels of pro-apoptotic genes, apoptosis-suppressing genes and invasion genes were determined by fluorescence quantitative PCR kit. Results: After treatment, the expression levels of SARI, Rab26, FHIT and eIF4E3 in cervical lesions of three groups were significantly higher than those before treatment, the content of serum CA125, TSGF and SCC-Ag and MRP8, MRP14, p38MAPK, ERK1 / The expression levels of c-myc and c-myc were significantly lower than those before treatment, and the expression of SARI, Rab26, FHIT and eIF4E3 in cervical cancer lesions were significantly higher than those in adjuvant chemotherapy group, endovascular radiotherapy group, serum CA125, TSGF, SCC -Ag content and MRP8, MRP14, p38MAPK, ERK1 / 2, c-myc expression were significantly lower than the adjuvant chemotherapy group, endovascular radiotherapy group. Conclusion: Preoperative intracavitary radiotherapy combined with neoadjuvant chemotherapy can promote the expression of apoptosis in stage Ⅱa and Ⅱb cervical cancer.
