目的探讨中药苦豆子总碱、苦参碱、苦参素、槐定碱对幽门螺杆菌(Helicobacter pylori,Hp)临床耐药株的体外抑菌作用。方法收集上消化道疾病患者胃黏膜标本,分离、鉴定出42株Hp临床菌株,采用E-test方法检测其对甲硝唑、克拉霉素、阿莫西林的耐药状况,共筛选出27株临床耐药株;采用液体倍比稀释法测定4种苦豆子生物碱对27株临床耐药株的体外抑菌效果。结果 42株临床分离的Hp菌株对甲硝唑、克拉霉素、阿莫西林的耐药率分别为64.3%、14.3%和0。苦豆子总碱、苦参碱、苦参素及槐定碱的MIC50分别为16、64、64、128 mg/ml。结论 Hp菌株对甲硝唑耐药率高,故甲硝唑不应作为根除Hp感染的一线药物。苦豆子总碱在体外抗Hp效果较好,有可能成为治疗Hp感染的一种可选药物。
Objective To investigate the antibacterial activity of total alkaloids of Sophora alopecuroides, matrine, oxymatrine and sophoridine on the clinical drug-resistant strains of Helicobacter pylori (Hp) in vitro. Methods Gastric mucosa specimens of patients with upper gastrointestinal diseases were collected and 42 strains of Hp strains were isolated and identified. The drug resistance status of metronidazole, clarithromycin and amoxicillin were tested by E-test. A total of 27 strains of Hp were screened out. Clinical drug-resistant strains; the antibacterial effect of 4 kinds of Sophora alopecuroides on 27 clinical drug-resistant strains in vitro was determined by liquid ratio dilution method. Results The resistance rates of 42 clinically isolated Hp strains to metronidazole, clarithromycin and amoxicillin were 64.3%, 14.3% and 0, respectively. Sophora alopecuroides, matrine, oxymatrine and sophoridine MIC50 were 16,64,64,128 mg / ml. Conclusion The Hp strain has a high resistance to metronidazole, so metronidazole should not be the first-line drug to eradicate Hp infection. Alkaloids of Sophora alopecuroides in vitro anti-Hp better effect, may become an optional drug for the treatment of Hp infection.
