目的:探讨血清和胃黏膜组织Ghrelin水平的影响因素,进一步明确其临床意义。方法:选择接受胃镜检查患者79例,于胃镜检查前抽取静脉血,应用ELISA法检测血清Ghrelin水平。所有患者进行胃镜检查时,于胃窦和胃体分别钳取1块组织行HE染色进行组织学评估;常规免疫组织化学方法检测胃黏膜组织Ghrelin表达水平,并行Warthin-Starry染色检测胃黏膜组织幽门螺杆菌(Hp)感染情况。结果:Hp阳性37例,Hp阴性42例;Hp阳性组血清Ghrelin水平与Hp阴性组差异不显著(P>0.05);胃窦部Hp阳性组Ghrelin表达水平显著高于Hp阴性组(387.78vs 239.15,P<0.05),而胃体部Hp阳性组Ghrelin表达水平与Hp阴性组差异不显著(P>0.05)。慢性浅表性胃炎(CSG)46例,慢性萎缩性胃炎(CAG)33例;CSG患者血清Ghrelin水平与CAG患者差异不显著(P>0.05);胃窦部及胃体部Ghrelin水平亦差异不显著(P>0.05)。男性患者血清Ghrelin水平非常显著低于女性患者(216.02vs 437.97,P<0.01);男性患者与女性患者胃窦部及胃体部Ghrelin表达水平均差异不显著(P>0.05)。结论:血清和胃黏膜组织Ghrelin水平与多种影响因素有关,须根据具体情况进行分析判断。
Objective: To investigate the influencing factors of Ghrelin level in serum and gastric mucosa and further clarify its clinical significance. Methods: 79 patients undergoing gastroscopy were selected. Venous blood was drawn before gastroscopy. Serum Ghrelin levels were measured by ELISA. All the patients underwent gastroscopy, and one piece of tissue was pliers-grafted in the gastric antrum and corpus respectively for histological evaluation. The expression of Ghrelin in gastric mucosa was detected by routine immunohistochemistry and the expression of pylorus in gastric mucosa was detected by Warthin-Starry staining Helicobacter (Hp) infection. Results: There were 37 Hp positive and 42 Hp negative. The serum Ghrelin level in Hp positive group was not significantly different from that in Hp negative group (P> 0.05). The expression of Ghrelin in Hp positive group was significantly higher than that in Hp negative group (387.78 vs 239.15 , P <0.05). However, the expression of Ghrelin in Hp positive group was not significantly different from that in Hp negative group (P> 0.05). There were 46 cases of chronic superficial gastritis (CSG) and 33 cases of chronic atrophic gastritis (CAG). The serum Ghrelin levels in CSG patients were not significantly different from those in CAG patients (P> 0.05). The levels of Ghrelin in gastric antrum and gastric body were also different Significant (P> 0.05). The Ghrelin level in male patients was significantly lower than that in female patients (216.02 vs 437.97, P <0.01). There was no significant difference in Ghrelin expression between gastric cancer and gastric mucosa between male patients and female patients (P> 0.05). Conclusion: The levels of Ghrelin in serum and gastric mucosa are related to various factors and should be analyzed and judged according to the specific conditions.
