心肌缺血损伤病人使用硝酸甘油期间维持血压对缺血心肌的作用仍有争论。本文观察17例有多支冠状血管严重病变(管腔狭窄75%以上)患者单独用硝酸甘油以及硝酸甘油与新福林合用时对左室功能,心肌能量和冠脉血流的效果。结果:舌下含服硝酸甘油0.4 mg 5分钟后,平均动脉压、左室充盈压、心脏指数和冠状窦血流减少(P<0.05)。10例紧接静脉滴注新福林(50~90μg/分,平均67μg/分),使平均动脉压升高至用硝酸甘油前的水平,其冠状窦血流、心肌灌注阶差及心脏效能指数(Cardiac Efficency Index)、冠状窦血流与心脏搏出量之比值均较之单独用硝酸甘油者有增加(P
The effect of maintaining blood pressure on ischemic myocardium during use of nitroglycerin in patients with myocardial ischemia remains controversial. In this paper, we observed the effects of nitroglycerin and nitroglycerin combined with neoprene on left ventricular function, myocardial energy and coronary blood flow in 17 patients with multiple coronary lesions (stenosis of more than 75%). Results: Mean arterial pressure, left ventricular filling pressure, cardiac index, and coronary sinus blood flow decreased after sublingual nitroglycerin 0.4 mg for 5 minutes (P <0.05).
