目的:探讨辅助生殖技术(ART)后的自然减胎和选择性减胎的差异。方法:回顾性分析早孕期间自然减胎151例和选择减胎140例病例,比较两组减胎时机、胚胎质量、ART方式、产科结局等。结果:两组年龄、不孕年限、减胎时妊娠天数等无差异。在自然减胎组中,①胚胎平均评分低于选择减胎组,分别为7.16±1.18 VS 7.44±0.94(P<0.05);②冷冻胚胎移植(FET)的所占比高于选择减胎组,分别为28.47%VS 16.43%(P<0.05);③减为双胎后的流产率低于选择性减胎组,分别为4.00%VS 23.89%(P<0.05)。减胎为单胎后流产率无差异。自然减胎组减胎时间为(51.75±9.40)天,选择减胎组为(50.71±5.36)天(P>0.05)。结论:选择性减胎适当延迟到自然减胎后,减少选择性减胎操作。如果移植的胚胎质量好,新鲜周期减少胚胎移植数目,从根本上减少多胎妊娠。
Objective: To explore the differences between natural and selective abortion after assisted reproductive technology (ART). Methods: 151 cases of spontaneous abortion and 140 cases of abortion during early pregnancy were retrospectively analyzed. The timing of abortion, quality of embryo, ART and obstetrics were compared between the two groups. Results: There was no difference between the two groups in terms of age, duration of infertility, number of days of pregnancy when aborted. In the natural ablation group, ① the mean embryo score was lower than that of the selective abortion group (7.16 ± 1.18 VS 7.44 ± 0.94, P <0.05); ② The proportion of frozen embryo transfer (FET) was higher than that of the selective abortion group , Respectively, 28.47% VS 16.43% (P <0.05). ③ The abortion rate after reduction to twin was lower than that of selective reduction group, 4.00% VS 23.89% (P <0.05). No difference in miscarriage rate after single birth. Natural abatement group abortion time (51.75 ± 9.40) days, select ablation group was (50.71 ± 5.36) days (P> 0.05). CONCLUSION: Selective delaying is delayed to a reasonable extent after natural ablation to reduce selective abortion. If the transplanted embryos are of good quality, fresh cycles reduce the number of embryos that are transplanted, radically reducing multiple pregnancies.
