目的应用超声弹性成像技术评价不同阶段下肢深静脉血栓(DVT)的弹性变化。方法根据临床症状和常规超声检查诊断为DVT的49例患者入选,根据患者症状开始时间将其分为急性期、亚急性期和慢性期血栓组。观察不同时期血栓的超声弹性图像特点,计算不同阶段血栓的应变比值(静脉周围肌肉组织/血栓)并进行比较。结果 (1)超声弹性成像显示,急性期血栓主要表现为红色,亚急性期血栓主要表现为绿色,慢性期血栓主要显示为蓝色。(2)慢性血栓组及亚急性血栓组的应变比值高于急性血栓组(P<0.001,P<0.05),慢性血栓组应变比值高于亚急性血栓组(P<0.05)。结论不同阶段血栓的超声弹性成像表现、血栓与周围肌肉组织的应变比值不同,超声弹性成像技术可用于评价不同阶段下肢深静脉血栓的硬度变化,对于血栓治疗方案的选择和预测并发症的发生具有一定的临床意义。
Objective To evaluate the elastic changes of deep venous thrombosis (DVT) in different stages by ultrasound elastography. Methods Forty-nine patients with DVT diagnosed according to clinical symptoms and conventional ultrasound were enrolled and divided into acute, subacute and chronic thrombosis groups according to their onset of symptoms. The characteristics of ultrasound elastography of thrombus in different periods were observed. The strain ratio of thrombus in different stages (perivascular muscle tissue / thrombus) was calculated and compared. Results (1) Elastography showed that acute thrombosis mainly showed red color, subacute thrombus mainly showed green color, and chronic phase thrombus mainly showed blue color. (2) The strain ratio of chronic thrombus group and subacute thrombus group was higher than that of acute thrombosis group (P <0.001, P <0.05). The strain ratio of chronic thrombus group was higher than that of subacute thrombus group (P <0.05). Conclusions Ultrasound elastography of thrombus at different stages shows different strain ratios of thrombus to surrounding muscle tissue. Ultrasound elastography can be used to evaluate the changes of the hardness of deep venous thrombosis of lower extremities in different stages. It has the following advantages for the selection of thrombus treatment and the occurrence of complications Certain clinical significance.
