本文研究慢性肾小球肾炎 (CGN )、原发性肾病综合征 (PNS )和慢性肾衰 (CRF )等慢性肾小球疾病患者外周血血小板P 选择素和淋巴细胞的细胞间粘附分子 1(ICAM 1)的变化 ,并探讨其与治疗效果之间的关系。采用流式细胞仪对 3 0例CGN、 3 4例PNS和 3 4例CRF的外周血血小板P 选择素和淋巴细胞ICAM 1的表达水平进行定量检测。结果 ,( 1)CGN、PNS和CGF患者的P 选择素阳性细胞百分率与正常对照组比较 ,有显著性差异 (P <0 0 1) ;CGN、PNS和CGF患者的ICAM 1阳性细胞百分率与正常对照组比较 ,也有显著性差异 (P <0 0 1) ;( 2 )CGN、PNS和CGF患者中病情好转者 ,治疗后的P 选择素阳性细胞百分率较治疗前有明显的下降 (P <0 0 5 ) ;而ICAM 1于PNS、CRF组有明显下降 (P <0 0 1) ,于CGN组无明显变化 (P >0 0 5 )。 3组患者中病情无明显好转者 ,治疗前后的P 选择素和ICAM 1的阳性细胞百分率 ,均无显著性差异 (P >0 0 5 )。CGN、PNS和CRF等慢性肾小球疾病患者P 选择素和ICAM 1的表达水平增高 ,提示P 选择素和ICAM 1参与了这些慢性肾小球疾病的病理过程 ;P 选择素和ICAM 1表达水平的高低还与这些慢性肾小球疾病的治疗效果有密切关系 ,因此它们还可作为在临床判断治疗效果的指标。
This study was to investigate the relationship between P-selectin and lymphocyte intercellular adhesion molecule-1 in peripheral blood of patients with chronic glomerular diseases such as chronic glomerulonephritis (CGN), primary nephrotic syndrome (PNS) and chronic renal failure (CRF) (ICAM 1) changes, and explore its relationship with the treatment effect. The expression of P-selectin and lymphocyte ICAM 1 in peripheral blood platelets of 30 CGN, 34 PNS and 34 CRF were detected by flow cytometry. Results: (1) The percentage of P-selectin positive cells in CGN, PNS and CGF patients was significantly different from that in control group (P <0.01). The percentage of ICAM 1 positive cells in CGN, PNS and CGF patients was significantly higher than that in normal controls (P <0.01). (2) The percentages of P-selectin-positive cells in CGN, PNS and CGF patients after treatment were significantly lower than those before treatment (P <0.01) However, ICAM 1 decreased significantly in PNS and CRF groups (P <0.01), but not in CGN group (P> 0.05). No significant improvement was found in the three groups of patients, P-selectin and ICAM 1 positive cells before and after treatment, there was no significant difference (P> 0.05). P-selectin and ICAM-1 were increased in patients with chronic glomerular diseases such as CGN, PNS and CRF, suggesting that P-selectin and ICAM-1 are involved in the pathogenesis of these chronic glomerular diseases.
