超声检查在儿童青少年脂肪肝诊断中的应用

目的探讨超声检查在儿童青少年脂肪肝诊断中的应用价值,为早期筛查脂肪肝提供方法。方法采用西门子G50彩色多普勒超声诊断仪,对北京市海淀区22所中小学校7~18岁儿童青少年2290名进行肝胆B超检查,同时进行身高和体重测量;根据超声影像学脂肪肝诊断标准,将检查学生分为脂肪肝组与非脂肪肝组,分析肝右叶最大斜径、门静脉主干内径与BMI及脂肪肝的关系。结果儿童青少年脂肪肝组BMI、肝右叶最大斜径、门静脉主干内径明显高于非脂肪肝组(P值均<0.01);肝右叶最大斜径值各年龄均是肥胖组(超重组(正常组(P值均<0.01);男生肝右叶最大斜径、门静脉主干内径均值高于女生,差异有统计学意义(P值均<0.01);在控制年龄、性别影响后,脂肪肝与BMI、肝右叶最大斜径、门静脉主干内径呈独立正相关(P值均<0.01)。结论超声检查对判断肝功能状态,选择治疗方案及预后判断有重要的参考价值。 Objective To investigate the value of ultrasonography in the diagnosis of fatty liver in children and adolescents and provide a method for early screening of fatty liver. Methods 2290 children aged 7-18 years old from 22 primary and secondary schools in Haidian District of Beijing were enrolled in this study. The liver and gallbladder ultrasonography was performed at the same time, height and weight were measured. According to diagnostic criteria of fatty liver by ultrasound imaging The students were divided into fatty liver group and non-fatty liver group. The maximum oblique diameter of the right hepatic lobe and the relationship between the diameter of the main portal vein and BMI and fatty liver were analyzed. Results In children and adolescents with fatty liver, BMI, maximum oblique diameter of the right lobe, and the diameter of the main portal vein were significantly higher than those of the non-steatosis group (all P <0.01) (P <0.01). The maximum oblique diameter of the right hepatic lobe and the mean arterial diameter of the portal vein in boys were significantly higher than those in the normal group (P <0.01). After controlling for age and gender, (P <0.01) .Conclusion Ultrasound has important reference value for judging the state of liver function, selecting the treatment plan and prognosis.
