目的:分析某军队医院门诊送检标本病原菌检出和耐药情况,为临床合理使用抗菌药物提供依据。方法:选择2014年1月-2015年12月门诊送检标本5774份,进行细菌培养及药物敏感性试验,并对致病菌和耐药情况进行分析。结果:在5774份送检标本中,共培养出病原菌2430株,占送检标本的42.09%。其中以痰液标本检出最多(63.38%),其后依次为血液、尿液、咽拭子、生殖道分泌物等。在培养出的2430株病原菌中,革兰阳性菌674株,占27.74%;革兰阴性菌1756株,占72.26%。革兰阳性菌中金黄色葡萄球菌、表皮葡萄球菌、肠球菌等对16种常用抗菌药物耐药试验,除对万古霉素、利奈唑胺和头孢哌酮舒巴坦敏感外,其余品种均有不同程度的耐药,其中对青霉素的耐药率最高。革兰阴性菌中鲍曼不动杆菌、大肠埃希菌、肺炎克雷伯、铜绿假单胞菌和阴沟肠杆菌等对21种常用抗菌药物均有较强的耐药性,其中对氨苄西林的耐药率最高。结论:某军队医院门诊细菌耐药性较高,应加强细菌耐药性监测和抗菌药物使用与管理。
Objective: To analyze the detection and drug resistance of pathogenic bacteria in the outpatient clinic of a military hospital and provide basis for the rational use of antimicrobial agents in clinic. Methods: From January 2014 to December 2015, 5774 samples from outpatient clinics were selected for bacterial culture and drug susceptibility testing. Pathogenic bacteria and drug resistance were analyzed. Results: Among the 5774 samples tested, 2430 strains of pathogens were co-cultivated, accounting for 42.09% of the samples tested. Among them, sputum samples were the most (63.38%), followed by blood, urine, throat swab, genital secretions and so on. Among the 2430 strains of pathogenic bacteria, 674 were Gram-positive bacteria, accounting for 27.74%. Gram-negative bacteria were 1756, accounting for 72.26%. Gram-positive bacteria Staphylococcus aureus, Staphylococcus epidermidis, enterococci and other 16 kinds of commonly used antimicrobial resistance test, in addition to vancomycin, linezolid and cefoperazone sulbactam sensitive, the other varieties have Different levels of resistance, of which the highest rate of resistance to penicillin. Gram-negative bacteria Acinetobacter baumannii, Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa and Enterobacter cloacae and other 21 kinds of commonly used antimicrobial drugs have strong resistance, which ampicillin The highest rate of resistance. Conclusion: The bacteriological resistance of a military hospital is high. Surveillance of bacterial resistance and use and management of antibacterials should be strengthened.
