目的探讨新生儿行为神经测定(NBNA)在发现新生儿高胆红素血症所致的轻微脑损伤中的应用价值。方法选取120例高胆红素血症新生儿作为观察组,在黄疸高峰期及消退期行NBNA测定,并与同期出生的60例正常新生儿作为对照组进行比较。结果观察组黄疸高峰期NBNA评分显著低于对照组,并随胆红素水平的增高而降低,黄疸消退后评分普遍提高,差异均具有统计学意义(P<0.05)。结论高胆红素血症可影响新生儿的神经行为能力,NBNA能及早发现新生儿高胆红素血症所致的轻微脑损伤,对早期干预治疗、改善预后具有重要意义,值得临床推广应用。
Objective To investigate the value of neonatal behavioral neurological assessment (NBNA) in the detection of mild brain injury caused by neonatal hyperbilirubinemia. Methods 120 neonates with hyperbilirubinemia were selected as the observation group. NBNA was measured in the peak and extinction period of jaundice and compared with 60 normal neonates born in the same period as the control group. Results The NBNA score of jaundice peak in observation group was significantly lower than that of control group, and decreased with the increase of bilirubin level. The score of jaundice subsided afterwards generally increased. The differences were statistically significant (P <0.05). Conclusions Hyperbilirubinemia can affect the neurobehavioral ability of neonates. NBNA can detect mild brain injury caused by neonatal hyperbilirubinemia early, which is of great significance for early intervention and prognosis, which is worthy of clinical application .
