关于ARDS的定义一般都采用Petty所提出的观点,即①有基础疾病;②胸片上出观弥漫性浸润影;③低氧血症(F_(IO_2)>0.6;P_(aO_2)<50mmHg);④肺楔压正常。但这一定义尚存有一些问题,如①在诊断上缺乏客观性;②不能定量地评价肺损伤程度;③虽然在临床上高度怀疑急性肺损伤,但因肺楔压稍高,或低氧血证未满足诊断标准而不能诊断等。对此,Murray等人提出肺损伤评分(lung injury score),试图对急性肺损伤程度定量化。
The definition of ARDS generally adopts the viewpoint put forward by Petty that ① there are basic diseases; ② diffuse infiltrates on chest radiograph; ③ hypoxemia (F_ (IO_2)> 0.6; P_ (aO_2) <50 mmHg); ④ wedge pressure normal. However, there are still some problems with this definition, such as: ① lack of objectivity in diagnosis; ② not quantitatively assess the extent of lung injury; ③ although highly suspected acute lung injury in the clinic, but slightly higher pulmonary wedge pressure, or hypoxia Blood test did not meet the diagnostic criteria can not be diagnosed. In response, Murray et al. Proposed a lung injury score to try to quantify the extent of acute lung injury.
