目的探讨钬激光在组合式输尿管软镜治疗肾结石中的应用技巧。方法肾结石患者68例,采用组合式软输尿管镜下钬激光碎石术治疗,光纤直径200μm,治疗功率8~30 W,能量0.8~1.5J,频率10~20 Hz。根据结石的软硬度和位置等情况进行边缘削切法或中央钻孔法粉碎结石。结果本组68例中65例进镜顺利,手术时间35~150 min,平均60.4 min。无大出血、输尿管穿孔等并发症。3例因输尿管扭曲狭窄无法置入输尿管镜。结论组合式输尿管软镜术中通过钬激光能量和碎石方法的选择等技巧,可以达到安全和高效的碎石效果。
Objective To investigate the application skills of holmium laser in combined ureteral soft lens in the treatment of kidney stones. Methods A total of 68 patients with nephrolithiasis were treated with combined ureteroscopic holmium laser lithotripsy. The diameter of the optical fiber was 200μm, the therapeutic power was 8-30 W, the energy was 0.8-1.5J, and the frequency was 10-20 Hz. According to the hardness and location of stones, etc. edge cutting or central drilling method to crush stones. Results 65 cases in this group of 65 cases went to the mirror smoothly, the operation time was 35 to 150 minutes, with an average of 60.4 minutes. No major bleeding, ureteral perforation and other complications. 3 cases of ureteral distortion due to ureteroscopy can not be placed. Conclusions The combined ureteral soft-lens technique achieves safe and efficient lithotripsy through techniques such as holmium laser energy and gravel selection.
