目的观察无创通气治疗COPD合并Ⅱ型呼吸衰竭的疗效。方法在常规治疗基础上应用BipAp呼吸机对100例COPD合并Ⅱ型呼衰患者进行通气治疗,并以同期病情相当患者100例鼻导管吸氧作对照观察。结果两组治疗后动脉血氧分压明显上升(P<0.01),但治疗组上升更高。治疗组血二氧化碳分压比治疗前明显下降,治疗前后相差非常显著(P<0.01)。对照组血二氧化碳分压治疗前后相差不显著(P>0.05)。治疗组疗程缩短。结论无创通气治疗COPD合并Ⅱ型呼吸衰竭疗效肯定,且无需气管插管或切开,并能缩短治疗时间。
Objective To observe the curative effect of noninvasive ventilation on COPD with type Ⅱ respiratory failure. Methods On the basis of routine treatment, 100 patients with COPD and type Ⅱ respiratory failure were treated with BipAp ventilator, and 100 patients with nasal catheter were enrolled in the same period. Results After treatment, arterial partial pressure of oxygen increased significantly (P <0.01), but the treatment group increased more. The partial pressure of carbon dioxide in treatment group decreased significantly compared with before treatment, and the difference was significant (P <0.01) before and after treatment. The control group before and after partial pressure of carbon dioxide treatment difference was not significant (P> 0.05). Treatment group shorter course of treatment. Conclusion Noninvasive ventilation is effective in treating COPD with type Ⅱ respiratory failure without tracheal intubation or incision and can shorten the treatment time.
