目的探讨前列腺癌表观弥散系数(ADC)值及容积转换常数(K~(trans))与前列腺癌病理分级(Gleason评分)的相关性,并对两者进行比较。方法回顾性分析病理证实的前列腺癌患者30例,根据病理结果中的Gleason评分将患者分为高分化、中分化及低分化组,比较组间ADC值及K~(trans)值的差异,并分析ADC值、K~(trans)值及Gleason评分间的相关性。结果三组前列腺癌ADC值及K~(trans)值组间整体差异均具有统计学意义(P<0.01)。ADC值与Gleason评分、K~(trans)值呈负相关(r=-0.633,P<0.01,r=-0.609,P<0.01),K~(trans)与Gleason评分呈正相关(r=0.579,P<0.01)。高分化组与中低分化组之间的ADC值及K~(trans)值差异均具有显著统计学意义(P<0.01),利用ROC曲线分析显示ADC值与K~(trans)值的曲线下面积分别为0.861、0.838,区分高分化组与中低分化组的敏感性和特异性分别为83.3%、88.9%,75.0%、77.8%。结论前列腺癌的ADC值及K~(trans)值与Gleason评分之间具有相关性,且ADC值与Gleason评分之间的相关性较K~(trans)值高,它们能够在一定程度上帮助预测前列腺癌的分化程度。
Objective To investigate the relationship between apparent diffusion coefficient (ADC) and volume transfer constant (K ~ (trans)) in prostate cancer and the pathological grading of prostate cancer (Gleason score), and to compare the two. Methods Thirty patients with pathologically confirmed prostate cancer were retrospectively analyzed. According to the Gleason score of the pathological findings, the patients were divided into well-differentiated, moderately differentiated and poorly differentiated groups. The differences of ADC value and K-trans value between the two groups were compared. The correlation between ADC value, K ~ (trans) value and Gleason score was analyzed. Results The overall differences between the ADC values and K-trans groups of three groups of prostate cancer were statistically significant (P <0.01). The ADC value was negatively correlated with Gleason score and K trans value (r = -0.633, P <0.01, r = -0.609, P <0.01) P <0.01). There was significant difference between ADC value and K ~ (trans) value between the well-differentiated group and the moderate-poorly differentiated group (P <0.01). The ROC curve analysis showed that under the curve of ADC value and K-trans value With the area of 0.861 and 0.838 respectively. The sensitivity and specificity of discriminating well-differentiated group from moderately-poorly differentiated group were 83.3%, 88.9%, 75.0% and 77.8% respectively. Conclusions There is a correlation between ADC value and K trans value of prostate cancer and Gleason score, and the correlation between ADC value and Gleason score is higher than K trans value, which can help predict to a certain extent Prostate cancer differentiation.
