目的探讨缬沙坦及氨氯地平除降压外对血管内皮功能及胰岛素抵抗的影响。方法选择40例1 -2级老年原发性高血压病患者随机分为缬沙坦与氨氯地平治疗组各20例,治疗12周两组降压达标后检测两组患者血浆一氧化氮(NO)、空腹血糖及空腹胰岛素浓度。结果两组降压总有效率分别为75%和80%,组间差异无显著性(P>0.05)。两组治疗后血浆NO均较治疗前升高,差异有显著性(P<0.05),空腹胰岛素水平较治疗前降低,胰岛素敏感指数较治疗前显著升高(P<0.05)。结论缬沙坦与氨氯地平降压作用肯定且相当,二者长期服用可改善血管内皮舒张功能及提高机体胰岛素的敏感性,改善胰岛素抵抗,尤适用于老年1-2级高血压合并糖尿病者。
Objective To investigate the effects of valsartan and amlodipine on vascular endothelial function and insulin resistance in vitro. Methods Forty patients with grade 1-2 senile essential hypertension were randomly divided into valsartan and amlodipine treatment group (n = 20). After treatment for 12 weeks, the levels of plasma nitric oxide NO), fasting blood glucose and fasting insulin concentration. Results The total effective rates of the two groups were 75% and 80% respectively, with no significant difference between the two groups (P> 0.05). Plasma NO in both groups was significantly higher than that before treatment (P <0.05). The fasting insulin level was lower than before treatment, and the insulin sensitivity index was significantly higher than before treatment (P <0.05). Conclusion Valsartan and amlodipine antihypertensive effect of certain and quite, both long-term use can improve vascular endothelial function and enhance the body’s insulin sensitivity, improve insulin resistance, especially for elderly with hypertension in patients with type 1-2 diabetes .
