目的探讨斜矢状位MRI增强扫描在肩袖损伤伴脂肪浸润程度中的诊断价值。方法回顾性分析自2014-10—2016-12手术治疗的36例肩袖损伤。术前由2位放射科医师在斜矢状位MRI图像上根据标准进行肩袖损伤脂肪浸润程度评分,取平均值。术中由术者及另外1名专业的运动医学医师在肩关节镜直视下对肩袖损伤断端进行仔细探查,并进行肩袖损伤脂肪浸润程度评分,取二者平均值。结果术前斜矢状位MRI肩袖损伤脂肪浸润程度评分为(2.02±0.13)分,术中肩袖损伤脂肪浸润程度的评分为(1.98±0.15)分,术前MRI评估与术中直视评估差异无统计学意义(t=-4.320,P=0.242)。结论斜矢状位MRI增强扫描可明确评估肩袖损伤的脂肪浸润程度,有助于选择正确的手术方式。
Objective To investigate the diagnostic value of oblique sagittal MRI enhanced scan in rotator cuff injury with fatty infiltration. Methods A retrospective analysis of 36 cases of rotator cuff injuries treated surgically from October 2014 to October 2016 was performed. Before surgery, radiosurgeons of rotator cuffs were scored by two radiologists on the oblique sagittal MRI images according to the standard and averaged. Surgery by the surgeon and another one of the professional sports physicians in the shoulder arthroscopy under the eyes of the rotarodal injury broken ends were carefully probed and rotiflorin injury fat infiltration score, whichever average. Results The degree of fat infiltration in rotator cuff injury was (2.02 ± 0.13) points and the degree of infiltration of rotator cuff was (1.98 ± 0.15) points in preoperative oblique sagittal MRI. The preoperative MRI and intraoperative direct vision The difference was not statistically significant (t = -4.320, P = 0.242). Conclusion The oblique sagittal MRI enhanced scan can clearly assess the extent of fat infiltration of rotator cuff injury and help to choose the correct surgical approach.
