目的探讨格拉斯哥昏迷记分法与高原脑水肿患者预后的关系。方法分析高原脑水肿病历资料,用格拉斯哥昏迷记分法对所有病例进行首次评分,根据评分结果将高原脑水肿患者分为四组Ⅰ组(GCS13分~15分)56例,Ⅱ组(GCS9分~12分)36例,Ⅲ组(GCS6分~8分)25例,Ⅳ组(GCS3分~5分)7例;并对肾功能、治愈天数、死亡例数进行统计。结果评分越低,病情越重,其肾功能损害的发生率越高,平均治愈天数也越长;第Ⅳ组(GCS3分~5分)7例患者中,死亡6例;第Ⅰ、Ⅱ、Ⅲ组无死亡病例。结论用格拉斯哥昏迷记分法可以较准确地判断高原脑水肿患者的病情程度;格拉斯哥昏迷记分越低,患者的预后越差。
Objective To investigate the relationship between Glasgow coma score and the prognosis of patients with high altitude brain edema. Methods The data of patients with cerebral edema in the plateau were analyzed. The first grade was scored by Glasgow Coma Scales. According to the results of the grading, the patients with plateau cerebral edema were divided into four groups: 56 cases in group Ⅰ (GCS13-15), group Ⅱ (GCS9 - 12 patients), 36 patients in group Ⅲ, 25 patients in group Ⅲ (GCS6 ~ 8) and 7 patients in group Ⅳ (GCS3 ~ 5). The numbers of renal function, days of cure and number of deaths were counted. The results of the lower score, the more serious the disease, the higher the incidence of renal dysfunction, the average number of days to cure the longer; group Ⅳ (GCS3 points to 5 points) in 7 patients, 6 patients died; Ⅰ, Ⅱ, Group Ⅲ no deaths. Conclusion Glasgow coma scoring method can more accurately determine the severity of patients with cerebral edema; Glasgow coma score is lower, the worse the prognosis of patients.
