目的:探讨Twist基因与p53蛋白在子宫颈鳞状细胞癌(cervical squamous cell carcinoma,SCC)及子宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)中的表达及其在癌变中的作用。方法:收集宫颈SCC组40例,CIN组85例(CIN Ⅰ级30例,Ⅱ级26例,Ⅲ级29例),正常宫颈鳞状上皮(正常对照组)26例。用自制组织芯片技术及免疫组织化学SP法检测Twist与p53在SCC组、CIN Ⅲ组、CIN Ⅱ组、CIN Ⅰ组中的表达状况,并与正常对照组比较。结果:正常对照组、CIN Ⅰ组、CIN Ⅱ组、CIN Ⅲ组及SCC组的Twist表达率分别为0,10.0%,42.3%,48.3%和52.5%;p53表达率分别为0,10.0%,46.2%,51.7%和52.5%;正常对照组Twist和p53的表达与CIN Ⅰ组、CIN Ⅱ组、CIN Ⅲ组及SCC组比较,差异均有统计学意义(均P<0.05)。SCC组Twist与p53呈正相关(r=0.683,P<0.05)。结论:Twist和p53随着CIN从轻到重至SCC,表达逐渐增强,表明两者共同参与了SCC癌变的发生、发展过程。联合检测Twist和p53对CIN分级、宫颈SCC的病理诊断具有重要意义。
Objective: To investigate the expression of Twist and p53 in cervical squamous cell carcinoma (SCC) and cervical intraepithelial neoplasia (CIN) and its role in carcinogenesis. Methods: 40 cases of cervical SCC, 85 cases of CIN (30 cases of CIN Ⅰ, 26 cases of Ⅱ, 29 cases of Ⅲ) and 26 cases of normal cervical squamous epithelium (normal control group) were collected. The expression of Twist and p53 in SCC group, CIN Ⅲ group, CIN Ⅱ group and CIN Ⅰ group were detected by home-made tissue microarray and immunohistochemical SP method, and compared with the normal control group. Results: Twist expression rates in normal control group, CIN Ⅰ group, CIN Ⅱ group, CIN Ⅲ group and SCC group were 0,10.0%, 42.3%, 48.3% and 52.5% respectively; the expression rates of p53 were 0,10.0% 46.2%, 51.7% and 52.5% respectively. The expression of Twist and p53 in normal control group was significantly different from that in CIN Ⅰ group, CIN Ⅱ group, CIN Ⅲ group and SCC group (all P <0.05). There was a positive correlation between Twist and p53 in SCC (r = 0.683, P <0.05). CONCLUSIONS: Twist and p53 gradually increase with the progression of CIN from light to heavy to SCC, indicating that they both participate in the carcinogenesis and progression of SCC. Joint detection of Twist and p53 on the CIN classification, pathological diagnosis of cervical SCC is of great significance.
