目的 应用多重荧光PCR分管技术及膜杂交技术分别对100 例疑似宫颈癌的门诊患者的子宫颈鳞柱状交界处上皮细胞样本进行HPV筛查,并对两种技术及结果进行比对研究.方法 应用多重荧光PCR分管技术对临床表现疑似宫颈癌的100 例患者进行21种基因型的筛查及病毒载量的标准化定量,应用膜杂交法对100 例患者进行21种基因型的筛查(两种技术分别筛查的21种基因型中有17种是相同的,4种是不同的),并以测序技术作为金标准,对上述两种技术的结果进行比对验证,并对两种技术的灵敏度、特异性等诊断效能指标进行对比.结果 通过多重荧光PCR分管技术检测出阳性病例为22 例,传统膜杂交法检测出阳性病例为13 例,有差异性结果的10 例患者样本经测序验证,均证实为感染了HPV病毒.利用SPSS软件对二者进行灵敏度、特异性、阳性预测值、阴性预测值的分析,数据分析证实两种技术方法差异具有统计学意义(P<0.05).结论 通过对比,发现多重荧光PCR分管技术在HPV筛查中有着更高的灵敏度、阳性检测率及阴性预测值,这更能够有效避免阳性病例的漏检,提高宫颈HPV感染的筛查水平,进而提高我国妇女宫颈癌的检出率.“,”Objective To perform HPV screening on epithelial cells of cervical squamous column junctions in 100 outpatients with suspected cervical cancer by multiplex PCR and tube hybridization techniques, and to compare the two techniques and results. Methods 21 genotypes and standardization of viral load were screened in 100 patients with clinically suspected cervical cancer using multiplex fluorescent PCR intubation technique. Screening of 21 genotypes in 100 patients was performed by membrane hybridization (17 of the 21 genotypes screened by the two technologies are identical and 4 are different), and the sequencing technology was used as the gold standard to verify the results of the above two techniques, and to compare diagnostic performance indicators such as sensitivity and specificity of the two techniques. Results 22 positive cases were detected by multiplex PCR, and 13 cases were detected by traditional membrane hybridization. 10 cases of differential results were verified by sequencing and confirmed to be infected with HPV. The sensitivity, specificity, positive predictive value and negative predictive value of the two methods were analyzed by SPSS software. The data analysis confirmed that the two methods were statistically different (P<0.05). Conclusion By comparison, it is found that multiplex PCR coupled tube technology has higher sensitivity, positive detection rate and negative predictive value in HPV screening, which can effectively avoid missed detection of positive cases and improve the screening level of cervical HPV infection, and can improve the detection rate of cervical cancer in women in China.
