目的:探讨俯卧位输尿管软镜联合微通道经皮肾镜同期治疗复杂性肾结石的有效性与安全性。方法:选取2017年7月至2019年3月于本院就诊的30例复杂性肾结石患者。其中15例患者一期均采取俯卧位输尿管软镜联合微通道经皮肾镜钬激光碎石术(双镜联合组),另外15例采取截石位单一微通道经皮肾镜钬激光碎石术治疗(对照组)。结果:双镜联合组手术时间[(148.33±14.95)min]高于对照组[(98.29±19.37)min],差异有统计学意义(n P0.05)。双镜联合组术后并发症发生率[13.3%(2/15)]与对照组[6.7%(1/15)]术后并发症的发生率比较,差异无统计学意义(n P>0.05)。双镜联合组结石清除率[93.3%(14/15)]高于对照组[60.0%(9/15)],差异有统计学意义(n P<0.05)。n 结论:输尿管软镜联合微通道经皮肾镜可提升结石清除率,具有良好的治疗效果。此类患者采用俯卧位具有操作简便,安全性高的优点,同期处理结石也有利于减轻患者的经济负担。“,”Objective:To evaluate the efficacy and safety of simultaneous treatment of complicated renal calculi on prone position by flexible ureteroscope combined with microchannel percutaneous nephroscopy.Methods:A total of 30 patients with complex kidney stones admitted to our hospital from July 2017 to March 2019 were enrolled.Among them, 15 cases were treated with flexible ureteroscopy combined with microchannel holmium laser lithotripsy.It was compared with 15 cases of complicated renal stones treated by percutaneous holmium laser lithotripsy.Results:The duration of surgery was(148.33±14.95) min and the length of hospital stay was (8.23±1.05) d in the double-mirror group, while the duration of surgery was (98.29±19.37) min and the length of hospital stay was(8.07±2.35) d in the control group.The double-lens group increased the duration of surgery, but did not increase the total number of days in hospital.The incidence of postoperative complications in the double-mirror group was 13.3%(2/15), which was not significantly different with that in the control group[6.7%(1/15)]. The stone clearance rate of the double-mirror group was 93.3%(14/15), higher than that of the control group 60.0%(9/15). The difference was statistically significant(n P<0.05).n Conclusions:In the treatment of complicated renal calculi, the combination of flexible ureteroscopy and microchannel percutaneous nephroscopy can improve the stone clearance rate, without increasing the incidence of postoperative complications, and has a good therapeutic effect.This kind of patients with prone position has the advantages of simple operation, high safety, and simultaneous treatment of stones that can also reduce the economic burden of patients.
