About 30% of colorectal cancer patients die within 2 years after curative resection. The authors have also found that 30% of patients who believe that the liver has no lesion during laparotomy have occult liver metastases. The purpose of this article is to determine whether this occult liver metastasis causes early death in patients. 35 consecutive colorectal cancer patients underwent curative resection and followed up for 2 years. Before and after the operation, 11 cases of occult liver metastases were detected by CT scan, and extensive hepatic lesions occurred afterwards. Nine cases died within 2 years. In the 24 cases, 2 cases died of liver diseases. According to the conventional Dukes classification criteria, 66% of patients with Grade B and 52% of C survived for 2 years. 11 cases with occult liver metastasis
