目的探讨锚定装置嵌顿的Günther Tulip下腔静脉滤器回收技巧及应用价值。方法2013年6月至2015年6月在西南医科大学附属中医医院接受Günther Tulip滤器回收术患者共37例,滤器留置时间98~117 d,平均96 d。采用单纯抓捕法、经股颈静脉双入路球囊扩张辅助技术回收滤器。结果采用单纯抓捕法顺利回收30例患者滤器,7例患者抓捕回收失败(5例为滤器锚定装置嵌顿,2例为锚定装置嵌顿伴回收钩贴壁),再辅以经股颈静脉双入路球囊扩张完成回收,回收成功率为100%。所有滤器取出后形态完整。结论抓捕法辅以经股颈静脉双入路球囊扩张术有助于提高锚定装置嵌顿的Günther Tulip滤器回收率,且患者损伤小,具有一定的临床应用价值。
Objective To investigate the recovery of Günther Tulip inferior vena cava filters with anchoring device and its application value. Methods From June 2013 to June 2015, 37 patients underwent Günther Tulip filter recuperation at the Traditional Chinese Medicine Hospital of Southwest Medical University. The filter retention time was 98-117 d (mean, 96 d). Simple catching method, double femoral jugular vein balloon dilatation assisted technology recovery filter. Results A total of 30 patients were recovered successfully by simple arresting method, 7 patients were recovered by catching failure (5 patients were incarcerated by filter anchors, 2 patients were incarcerated by anchoring device), supplemented by Femoral vein dual approach balloon dilatation to complete the recovery, recovery success rate was 100%. All filters removed after the form of integrity. Conclusions Catgut apprehension assisted by the double jugular vein approach of femoral vein dilation can help to improve the recovery rate of Günther Tulip incarcerated by the anchoring device, and has a small clinical application value.
