目的总结脑转移瘤的MRI平扫及增强扫描的特点,并对所有病例行弥散加权成像,探讨其在诊断脑转移瘤中的应用价值。方法回顾分析来我院检查的脑转移瘤患者31例,均经病理或临床证实,并接受MRI平扫及增强扫描。结果31例患者平扫发现病灶48个,增强后发现病灶110个。病灶位于幕上27例,幕下6例,幕上幕下并存2例,病灶主要表现为环形或结节状强化。分析57个直径大于1cm的病灶弥散加权成像表现,其中41个实质性病灶中,14个呈高信号,27个呈高、低混杂信号;其中16个病灶为囊变,中央囊变区均表现为低信号,环壁呈高信号。结论脑转移瘤行MRI增强扫描可明显提高病灶的检出率,并能明确病灶的边界,结合常规MR序列,可明显提高诊断的准确性。
Objective To summarize the characteristics of brain metastases by MRI and enhanced scan, and to perform diffusion weighted imaging in all cases to explore its value in the diagnosis of brain metastases. Methods Retrospective analysis of 31 patients with brain metastases examined in our hospital were confirmed by pathology or clinical examination and underwent MRI plain scan and enhanced scan. Results In 31 cases, 48 lesions were found by plain scanning and 110 lesions were found by enhancement. Lesions were located in the top 27 cases, 6 cases under the screen, 2 cases co-exist under the screen, the lesion is mainly manifested as ring or nodular enhancement. Fifty-seven lesions of more than 1 cm in diameter were analyzed by diffusion-weighted imaging. Among 41 substantive lesions, 14 showed high signal and 27 showed high and low mixed signals. Among them, 16 lesions were cystic and cystic areas For the low signal, ring wall was high signal. Conclusion MRI enhanced scan of brain metastases can significantly improve the detection rate of lesions, and can clear the border of lesions, combined with conventional MR sequences, can significantly improve the diagnostic accuracy.
