目的 :在调查不同等级医院合理用药现状基础上探索推进合理用药的工作模式。方法 :选择 7家不同等级医院2年各 5个月门诊处方和当日门诊就医患者 ,采取多中心、随机、平行对照试验方法 ,对合理用药国际指标的核心指标进行干预前后现场调查 ,并用SPSS10 .0软件进行统计分析。结果 :干预组自身对比平均用药品种数从 2 .73种降到 2 .36种(P <0 .0 1) ;抗生素使用百分率从 37.7%降到 33.7% ;针剂使用百分率从 2 0 .0 %降到 16 .3% ;国家基本药物使用百分率从73.4 %上升到 82 .5 % (P <0 .0 1) ;药品标示完整率从97.8%上升到 10 0 .0 % ;患者了解正确用药方法百分率从88.1%上升到 94 .5 % ,干预后指标好于干预前。两组组间对比平均用药品种数、抗生素使用百分率、基本药物使用百分率、药品标示完整率以及患者了解正确用法方法百分率干预组均好于非干预组。结论 :采取宣传教育 (针对医生、药师、管理人员、患者 )、加强管理、贯彻规章制度等措施 ,对于推进合理用药具有可操作性和有效性。
OBJECTIVE: To explore the working mode of promoting rational drug use on the basis of investigating the current status of rational drug use in hospitals in different grades. Methods: Seven outpatient prescriptions and outpatients were selected from 7 hospitals of different grades for 2 years for 2 years. A multicentre, randomized, parallel controlled trial was conducted to investigate the core indicators of rational use of drugs before and after the intervention and SPSS10. 0 software for statistical analysis. Results: Compared with the control group, the average number of drug users in the intervention group dropped from 2.73 to 2.36 (P <0.01), the percentage of antibiotics decreased from 37.7% to 33.7%, and the percentage of injections from 20.0% To 16.3%; the percentage of national essential drugs increased from 73.4% to 82.5% (P <0.01); the labeled rate of drugs increased from 97.8% to 100.0%; patients knew the correct medication method The percentage increased from 88.1% to 94.5%, after intervention indicators better than before intervention. The average number of patients, the percentage of antibiotics used, the percentage of essential drugs used, the percentage of drug labeled intact, and the percentages of patients treated with the correct usage method in the two groups were better than those in the non-intervention group. Conclusion: Measures such as publicity and education (for doctors, pharmacists, managers and patients), strengthening management and implementing rules and regulations are feasible and effective in promoting rational drug use.
