目的探讨含司肤泰克(Safetac)的超薄透明膜(美菲)对鼻咽癌患者颈部急性放射性皮炎的防护效果,为临床预防提供参考。方法选取2013年10月-2014年10月在医院肿瘤诊疗中心行放射治疗的46例鼻咽癌患者为研究对象,将患者分为对照组22例、试验组24例,在同样常规皮肤护理的基础上,对照组和试验组分别使用三乙醇胺乳膏和美菲保护放射野皮肤,使用RISRAS和RTOG评分表评估皮肤反应的严重程度。结果 RTOG评分显示,试验组33.3%的患者皮肤无任何变化,58.4%出现Ⅰ级反应,8.3%为Ⅱ级反应,无Ⅲ、Ⅳ级反应,对照组18.2%出现Ⅰ级反应,45.5%出现Ⅱ级反应,27.3%出现Ⅲ级反应,9.1%出现Ⅳ级反应,试验组和对照组的术后RTOG评分比较,差异有统计学意义(U=46.000,P<0.01);RISRAS评分显示,与使用三乙醇胺乳膏的对照组比较,美菲敷料显著降低了试验组中患者的RISRAS评分,而且随着放疗剂量的增加,患者的皮肤反应及主观症状均保持在较低评分。结论美菲在首次放疗前及全过程中使用,可以有效降低急性放射性皮炎的发生。
Objective To investigate the protective effect of ultrafine transparent membrane (Meifei) containing Safetac on the acute radiation dermatitis in the neck of patients with nasopharyngeal carcinoma (NPC), so as to provide a reference for clinical prevention. Methods Forty-six patients with nasopharyngeal carcinoma treated by radiotherapy from October 2013 to October 2014 in our hospital were divided into control group (n = 22) and experimental group (n = 24). In the same routine skin care On the basis of the control group and the test group, the triethanolamine cream and the mefloxacin were respectively used to protect the radiation skin, and the severity of the skin reaction was evaluated using the RISRAS and RTOG scoring tables. Results The RTOG score showed that there was no change in the skin of 33.3% of patients in the test group, Ⅰ grade reaction in 58.4%, Ⅱ grade reaction in 8.3%, Ⅲ grade and Ⅳ grade reaction in 18.3%, Ⅰ grade reaction in 18.2% and 45.5% Grade reaction, grade Ⅲ reaction occurred in 27.3% and grade Ⅳ reaction in 9.1%. The postoperative RTOG score of test group and control group was statistically significant (U = 46.000, P <0.01); RISRAS score showed that, Compared with the control group of triethanolamine cream, the American-Philippine dressing significantly reduced the RISRAS score of patients in the experimental group, and patients’ skin reactions and subjective symptoms were kept at a lower score as the dose of radiotherapy increased. Conclusion US and Philippine before the first radiotherapy and the whole process, can effectively reduce the incidence of acute radiation dermatitis.
