感染性疾病是新生儿期的常见病和多发病,由于新生儿期患儿免疫功能低下,起病紧急,一旦发病往往病情危重,预后较差,因此早期作出正确诊断极其重要性。目前临床上检测新生儿感染的标志物很多,主要包括血常规、C反应蛋白(CRP)、血清降钙素原(PCT)、白细胞介素-6(IL-6)等。血清PCT检测对于早期新生儿细菌感染特别是严重感染的敏感度、特异性高,且可以反映感染的严重程度,以及用药的疗效和预后,故降钙素检测可以作为临床早期诊断细菌感染特别是严重细菌感染性疾病的指标。IL-6是参与多种生理与免疫过程的重要细胞因子,具有广泛的生物活性,目前认为它是参与全身炎症反应综合征(SIRS)、脓毒症等的重要炎性介质。IL-6可作为早期判断新生儿细菌感染的指标。临床上,如果将血清PCT与IL-6联合检测,就可以提高新生儿感染性疾病诊断的早期准确率。本文就血清IL-6和PCT在早期新生儿感染性疾病中的变化规律及临床意义做一综述。
Infectious diseases are common and frequently-occurring diseases in the neonatal period. Due to the immunocompromised children in the neonatal period, the onset of the disease is urgent. At present, there are many biomarkers of neonatal infection in clinic, including blood routine test, C-reactive protein (CRP), serum procalcitonin (PCT) and interleukin-6 (IL-6) Serum PCT detection for early neonatal bacterial infections, especially severe infections with high sensitivity and specificity, and can reflect the severity of the infection, as well as the efficacy and prognosis of the drug, so calcitonin detection can be used as early clinical diagnosis of bacterial infections, especially Serious bacterial infectious disease indicators. IL-6, an important cytokine involved in many physiological and immune processes, has a wide range of biological activities and is currently considered as an important inflammatory mediator involved in systemic inflammatory response syndrome (SIRS), sepsis and the like. IL-6 can be used as an early indicator of neonatal bacterial infection. Clinically, serum PCT combined with IL-6 can improve the early diagnosis of neonatal infectious disease. This article reviews the changes and clinical significance of serum IL-6 and PCT in early neonatal infectious diseases.
