目的探讨心肌肌钙蛋白Ⅰ(cTn-Ⅰ)在新生儿缺氧缺血性脑病(HIE)心肌损伤中的诊断价值。方法测定38例HIE患儿的血清cTnⅠ、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平,并与30例健康的新生儿(对照组)检测结果比较。结果在HIE时血清cTnⅠ水平高于正常,且中度与重度HIE血清cTnI水平明显高于对照组,差异均有统计学意义(P<0.01);血清cTnⅠ在诊断HIE患儿心肌损伤时阳性率较血清CK、CK-MB更为敏感(P<0.05),具有统计学意义。结论 HIE患儿病情愈重,其心肌损伤程度愈重,血清cTnⅠ水平愈高。
Objective To investigate the diagnostic value of cardiac troponin Ⅰ (cTn-Ⅰ) in myocardial injury of neonatal hypoxic-ischemic encephalopathy (HIE). Methods Serum levels of cTnⅠ, creatine kinase (CK) and creatine kinase (CK-MB) in 38 children with HIE were measured and compared with those of 30 healthy newborns (control group). Results Serum levels of cTnⅠ in HIE were higher than those in normal controls, and levels of cTnI in moderate and severe HIE were significantly higher than those in control group (P <0.01). The positive rate of serum cTnⅠ in diagnosis of myocardial injury in children with HIE More sensitive than serum CK, CK-MB (P <0.05), with statistical significance. Conclusions The more severe the disease in children with HIE, the more the degree of myocardial injury, the higher the level of serum cTn Ⅰ.
