目的:探讨高度宫颈上皮内瘤变患者宫颈锥切术对妊娠及孕期宫颈管长度的影响。方法:收集我院收治的30例欲行宫颈锥切术的高度宫颈上皮内瘤变患者为实验组,对照组选取30例同期在妇科门诊行孕前检查的的妇女。对实验组患者行宫颈锥切术。治疗结束后对实验组患者的宫颈管长度和实验组患者手术后成功妊娠与同期对照组妊娠妇女的宫颈管长度、妊娠情况以及妊娠结局进行检测并比较。结果:实验组患者治疗后宫颈管长度与治疗前相比较低(P<0.05),实验组患者成功妊娠后妊娠12周、20周、28周宫颈管长度相比无明显差异(P>0.05),实验组患者手术后成功妊娠率较对照组显著降低(P<0.05),妊娠早产胎膜早破、脐带绕颈早产以及足月分娩率与对照组相比无明显差异(P>0.05)。结论:高度宫颈上皮内瘤变患者宫颈锥切术后宫颈管长度减小,成功妊娠率较低,孕期宫颈管长度以及早产率与正常孕妇相比无差异。
Objective: To investigate the effect of cervical conization on the length of cervical canal during pregnancy and pregnancy in patients with highly cervical intraepithelial neoplasia. Methods: Thirty patients with high grade cervical intraepithelial neoplasia who underwent cervical conization were enrolled in this study. The control group was selected from 30 women who underwent gynecological clinic for prenatal examination. Patients in the experimental group underwent cervical conization. At the end of treatment, the cervical canal length of the experimental group and the cervical canal length, pregnancy status and pregnancy outcome of the pregnant women in the experimental group and the control group after successful operation were compared and compared. Results: The length of cervical canal in the experimental group was significantly lower than that before treatment (P <0.05). There was no significant difference in the cervical canal length between the experimental group and the pregnant group at 12 weeks, 20 weeks and 28 weeks after successful pregnancy (P> 0.05) . The success rate of pregnancy after operation in experimental group was significantly lower than that in control group (P <0.05). Premature rupture of membranes, premature umbilical cord around neck and full-term delivery rate had no significant difference compared with control group (P> 0.05). Conclusions: The cervical canal length decreases after cervical conization in patients with high grade cervical intraepithelial neoplasia. The successful pregnancy rate is lower, the length of cervical canal during pregnancy and the rate of premature birth are not different from those of normal pregnant women.
