目的探讨980nm半导体红激光汽化术治疗高危高龄前列腺增生症(BPH)的疗效及安全性。方法选取100例高危高龄BPH患者进行红激光汽化术,观察平均手术时间、术中出血情况、术后留置导尿时间及最大尿流率(Qmax)、国际前列腺症状评分(IPSS)、生活质量评分(QOL)及残余尿量(PVR)等指标并进行评价分析。结果 100例患者均顺利完成手术,手术时间平均(40±25)min,术中出血少,无输血病例,术后留置导尿管时间平均(40±18)h,术后1月随访,IPSS评分(7.6±5.8)分、QOL(3.2±1.4)分、Qmax(14.6±4.8)mL/s、PVR(41.7±39.2)mL,较术前均有明显好转,差异有统计学意义(均P<0.01),术后随访3个月,上述指标较术前相比均有明显好转,差异有统计学意义(均P<0.01)。无严重尿失禁、继发性出血或尿潴留需再次手术病例。结论 980nm半导体红激光汽化术治疗高危高龄BPH是安全有效的。
Objective To investigate the efficacy and safety of 980nm semiconductor red laser vaporization in the treatment of high-risk benign prostatic hyperplasia (BPH). Methods 100 cases of high-risk BPH patients were treated by red laser vaporization. The mean operative time, intraoperative bleeding, postoperative catheterization time and maximal flow rate (Qmax), International Prostate Symptom Score (IPSS), quality of life score (QOL) and residual urine output (PVR) and other indicators and evaluation analysis. Results All the 100 patients completed the operation successfully. The average operation time was (40 ± 25) min. There were less intraoperative bleeding, no transfusion cases, and the average duration of indwelling catheterization was (40 ± 18) h. The patients were followed up for 1 month, (7.6 ± 5.8), QOL (3.2 ± 1.4), Qmax (14.6 ± 4.8) mL / s and PVR (41.7 ± 39.2) mL, respectively, which were significantly improved compared with those before operation (all P <0.01). All patients were followed up for 3 months. The above indexes were significantly improved compared with those before operation (all P <0.01). No serious urinary incontinence, secondary bleeding or urinary retention need surgery again. Conclusion The 980nm semiconductor red laser vaporization is safe and effective in the treatment of high-risk BPH.
