目的探讨3.0 T高分辨力MRI T_2WI对Ⅱ期宫颈癌精确分期的应用价值。方法回顾性分析经手术病理证实的32例Ⅱ期宫颈癌患者的MRI影像资料,以术后病理为金标准,比较术前临床FIGO分期与MRI分期的诊断效能。结果临床分期对Ⅱ期宫颈癌精确分期的敏感度、特异度、准确率、阳性预测值、阴性预测值分别为76.9%、68.4%、71.9%、62.5%、81.3%;MR分期分别为84.6%、94.7%、90.6%、91.7%、90.0%。两种分期方法受试者工作特征曲线(ROC)曲线下面积分别为临床分期0.727、MRI分期0.879,差异有显著统计学意义(Z=2.190,P<0.05)。结论 3.0 T高分辨力MRI T_2WI对于Ⅱ期宫颈癌精确分期有较高的准确性及特异性,应当作为宫颈癌术前MR检查的常规扫描序列。
Objective To investigate the value of 3.0 T high resolution MRI T_2WI in the accurate staging of stage Ⅱ cervical cancer. Methods The MRI images of 32 patients with stage Ⅱ cervical cancer confirmed by surgery and pathology were retrospectively analyzed. The pathological features of the images were taken as gold standard, and the diagnostic efficacy of FIGO staging and MRI staging were compared. Results The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of clinical staging were 76.9%, 68.4%, 71.9%, 62.5% and 81.3% respectively for the stage Ⅱ cervical cancer; MR staging was 84.6% , 94.7%, 90.6%, 91.7%, 90.0%. The areas under the curve of ROC curve for the two staging methods were 0.727 for clinical stage and 0.879 for MRI stage, respectively. The difference was statistically significant (Z = 2.190, P <0.05). Conclusion The 3.0 T high resolution MRI T 2 WI has high accuracy and specificity for the accurate staging of stage Ⅱ cervical cancer and should be used as a routine scan sequence for preoperative MR examination of cervical cancer.
