目的 探讨膀胱移行细胞癌中环氧化酶 -2 (Cyclooxygenase -2 ,COX -2 )表达及其与肿瘤细胞增殖活性和凋亡程度的关系。方法 采用免疫组织化学法检测 5 4例膀胱癌、2 9例癌旁组织和 10例正常膀胱黏膜中COX -2和Ki67的表达 ,并采用TUNEL法检测原位细胞凋亡水平 ,结合临床病理资料分析。结果 膀胱癌组织中COX -2阳性表达、Ki67标记指数及细胞凋亡指数均显著高于癌旁组织及正常膀胱黏膜 (P <0 0 1) ;COX -2表达水平和Ki67标记指数均与膀胱癌分化、临床分期、淋巴转移呈正相关 ;细胞凋亡指数与肿瘤分期、分级呈负相关 ,即肿瘤“瘤级”和“瘤期”越高 ,则细胞凋亡越少 ,易复发肿瘤其细胞凋亡指数明显低于无复发肿瘤 ;COX -2表达水平的升高伴随着肿瘤细胞增殖活性的增加和凋亡程度的降低。结论 膀胱移行细胞癌中COX -2表达上调 ,其表达水平与肿瘤细胞的分化、生长及转移相关 ;COX -2的高表达可能通过促进细胞增殖和 /或诱导细胞发生凋亡耐受在膀胱癌的发生、发展中发挥重要作用
Objective To investigate the expression of Cyclooxygenase-2 (COX-2) in transitional cell carcinoma of the bladder and its relationship with the proliferation activity and apoptosis of tumor cells. Methods Immunohistochemistry was used to detect the expression of COX-2 and Ki67 in 54 cases of bladder cancer, 29 cases of paracancerous tissues and 10 cases of normal bladder mucosa. TUNEL method was used to detect the level of apoptosis in situ and combined with clinicopathological data analysis. Results The positive expressions of COX-2, Ki67 and apoptosis index in BTCC were significantly higher than those in para-cancerous tissues and normal bladder mucosa (P <0.01). The expressions of COX-2 and Ki67 in index of bladder cancer Cancer differentiation, clinical stage and lymph node metastasis. There was a negative correlation between apoptosis index and tumor stage and grade, that is, the higher tumor grade and tumor stage, the less cell apoptosis, The apoptotic index was significantly lower than that of the non-recurrent tumor. The increase of COX-2 expression accompanied with the increase of tumor cell proliferation activity and the decrease of apoptosis. Conclusions The expression of COX-2 is up-regulated in transitional cell carcinoma of the bladder and its expression level is related to the differentiation, growth and metastasis of BTCC.
