目的探讨基于细胞体外培养的干扰素释放试验对结核性胸膜炎辅助诊断的价值。方法采集结核性胸膜炎患者33例、非结核性胸膜炎组36例患者静脉血,应用结核分枝杆菌γ-扰素体外释放(TB-IGRA)诊断试剂检测静脉血细胞体外刺激培养后干扰素释放水平,同时抽取胸水常规检测腺苷脱氨酶(ADA),并与该常规方法进行比较。结果以全血细胞为检测样本体外干扰素释放试验对结核性胸膜炎检测的灵敏度93.9%,特异性88.9%,准确度91.3%;经比较干扰素释放试验灵敏度高于常规ADA检测方法(75.8%),差异有统计学意义(P<0.05)。结论基于细胞体外培养的干扰素释放试验检测方便、快捷,有助于提高对结核性胸膜炎的诊断效率。
Objective To investigate the value of interferon release assay based on cell culture in vitro in the diagnosis of tuberculous pleurisy. Methods Thirty-three patients with tuberculous pleurisy and 36 patients with non-tuberculous pleurisy were enrolled in the study. The levels of interferon in cultured venous cells were detected by TB-IGRA diagnostic reagent in vitro. Concurrently, pleural effusion was routinely assayed for adenosine deaminase (ADA) and compared with this routine method. Results The sensitivity, specificity and accuracy of interferon release assay were 93.9%, 88.9% and 91.3%, respectively. The sensitivity of interferon release assay was higher than that of conventional ADA assay (75.8%), The difference was statistically significant (P <0.05). Conclusion The detection of interferon release based on cell culture in vitro is convenient and quick, which is helpful to improve the diagnostic efficiency of tuberculous pleurisy.
