目的分析血清C反应蛋白在小儿肺炎诊治中的诊断价值。方法选取永修县人民医院2013年10月—2015年10月收治的小儿肺炎患儿172例,按照疾病类型不同分为支原体肺炎组56例、细菌性肺炎组50例、病毒性肺炎组66例,另选取本院同期健康体检儿童45例作为对照组。比较4组受试者治疗前后C反应蛋白水平及3组患儿C反应蛋白阳性检出情况。结果治疗前,4组受试者C反应蛋白水平比较,差异有统计学意义(P<0.05);支原体肺炎组、细菌性肺炎组患儿C反应蛋白水平高于对照组(P<0.05);病毒性肺炎组与对照组C反应蛋白水平比较,差异无统计学意义(P>0.05)。治疗后,4组受试者C反应蛋白水平比较,差异有统计学意义(P<0.05);治疗后细菌性肺炎组C反应蛋白水平高于支原体肺炎组、病毒性肺炎组(P<0.05);治疗后支原体肺炎组、细菌性肺炎组C反应蛋白水平低于治疗前(P<0.05);治疗前后病毒性肺炎组C反应蛋白水平比较,差异无统计学意义(P>0.05)。3组患儿C反应蛋白阳性检出率比较,差异有统计学意义(P<0.05);细菌性肺炎组C反应蛋白阳性检出率为100.0%,高于支原体肺炎组的78.6%和病毒性肺炎组的18.2%(P<0.05)。结论血清C反应蛋白水平在小儿肺炎临床诊治中诊断及鉴别价值较高。
Objective To analyze the diagnostic value of serum C-reactive protein in diagnosis and treatment of pediatric pneumonia. Methods 172 patients with pediatric pneumonia admitted to Yongxian County People’s Hospital from October 2013 to October 2015 were divided into 56 cases of mycoplasma pneumonia group, 50 cases of bacterial pneumonia group, 66 cases of viral pneumonia group, Another 45 cases of healthy children in our hospital were selected as control group. The levels of C-reactive protein in the four groups before and after treatment were compared with those in the three groups. Results Before treatment, the levels of C - reactive protein in four groups were significantly different (P <0.05). The levels of C - reactive protein in children with mycoplasma pneumonia and bacterial pneumonia were higher than those in control group (P <0.05). Viral pneumonia group and control group C-reactive protein levels, the difference was not statistically significant (P> 0.05). After treatment, the C-reactive protein levels of the four groups of subjects were significantly different (P <0.05); the C-reactive protein level in bacterial pneumonia group was higher than that in mycoplasma pneumonia group and viral pneumonia group (P <0.05) (P <0.05). The level of C-reactive protein in mycoplasma pneumonia group and bacterial pneumonia group after treatment was lower than that before treatment (P <0.05). There was no significant difference in C-reactive protein between viral pneumonia group before and after treatment (P> 0.05). The positive rate of C-reactive protein in the three groups was statistically significant (P <0.05). The positive rate of C-reactive protein in the bacterial pneumonia group was 100.0%, higher than 78.6% in the mycoplasma pneumonia group and the viral Pneumonia group 18.2% (P <0.05). Conclusion The serum C-reactive protein level is of high value in the diagnosis and differential diagnosis of pneumonia in children.
