目的探讨在系统性红斑狼疮(SLE)患者中,贫血和炎症对红细胞RDW产生的影响。方法纳入2011年1月~2013年7月海口市人民医院确诊为系统性红斑狼疮的患者43例,入院后次日清晨空腹,静息,平卧位无菌从肘静脉采血分析,以血红蛋白(HGB)<110g/L作为贫血诊断标准,超敏C反应蛋白(Hs-CRP)用免疫比浊法分析,建立临床资料数据库并对数据进行统计学分析。结果 43例SLE患者HGB、MCV、MCH、MCHC明显低于正常人(P<0.001),RDW、Hs-CRP明显高于正常人(P<0.01)。25例贫血SLE患者HGB、MCV、MCH、MCHC明显低于正常人和非贫血SLE患者(P<0.05),而其RDW和Hs-CRP值高于正常人和非贫血SLE患者(P<0.05)。SLE患者和SLE贫血患者HGB与RDW存在负相关(P<0.05),SLE非贫血患者、正常人HGB与RDW无显著相关性(P>0.05),各组Hs-CRP与RDW均无显著相关性(P>0.05)。结论 SLE患者贫血与炎症均可对其RDW产生影响,但是贫血影响更大,且贫血与RDW之间存在负相关。
Objective To investigate the effects of anemia and inflammation on erythrocyte RDW production in patients with systemic lupus erythematosus (SLE). Methods Forty-three patients diagnosed as systemic lupus erythematosus were selected from Haikou People’s Hospital from January 2011 to July 2013. After fasting and resting in the morning after admission, HGB) <110g / L as diagnostic criteria for anemia, high-sensitivity C-reactive protein (Hs-CRP) by immunoturbidimetric assay to establish a clinical data database and the data were statistically analyzed. Results The HGB, MCV, MCH and MCHC in 43 SLE patients were significantly lower than those in normal controls (P <0.001). The RDW and Hs-CRP were significantly higher than those in normal controls (P <0.01). The serum levels of HGB, MCV, MCH and MCHC in 25 anemic SLE patients were significantly lower than those in normal and non-anemic SLE patients (P <0.05), while the values of RDW and Hs-CRP were higher in SLE patients with anemia than those in normal and non-anemic SLE patients . There was a negative correlation between HGB and RDW in SLE patients and SLE anemia patients (P <0.05). There was no significant correlation between HGB and RDW in non-anemia patients and normal subjects (P> 0.05) (P> 0.05). Conclusion Anemia and inflammation in patients with SLE may have an impact on RDW, but anemia has a greater impact, and there is a negative correlation between anemia and RDW.
