目的评价珊瑚羟基磷灰石人工骨用于良性骨肿瘤切除后重建的效果,并与异体松质骨作比较。方法整理从2004年12月至2008年7月我院使用珊瑚羟基磷灰石与异体松质骨植骨的两组病例各62例。两组病例在治疗时间、年龄、性别、病种、病灶部位、植骨量和随访时间等方面均匹配良好。通过影像学和临床随访,评估并比较植骨的愈合速度和愈合率,分析影响人工骨愈合的因素。结果羟基磷灰石的初步愈合率为62.9%,异体骨的初步愈合率为96.8%,差异有统计学意义(P=0.000);羟基磷灰石的平均初步愈合时间为(13.3±7.5)个月,异体骨平均初步愈合时间为(8.8±4.9)个月,差异有统计学意义(P=0.000)。性别(P=0.369)、年龄(P=0.290)、病灶部位(P=0.704)、病种(P=0.994)和植骨量(P=0.348)均不是影响羟基磷灰石愈合情况的显著因素。羟基磷灰石组与异体骨组患者术后MSTS平均分分别为(28.8±1.8)分和(29.0±1.5)分,两组无显著差异(P=0.450)。结论羟基磷灰石用于良性骨肿瘤切除后骨缺损的重建,术后功能满意,但愈合率和愈合速度均低于异体骨。
Objective To evaluate the effect of Coral hydroxyapatite artificial bone for resection of benign bone tumors after resection and compare with allogenic cancellous bone. Methods From December 2004 to July 2008 in our hospital coral hydroxyapatite and allogeneic cancellous bone graft in two groups of 62 cases. The two groups of patients in the treatment time, age, gender, disease, lesion site, bone mass and follow-up time and so on are good. Through imaging and clinical follow-up, the healing rate and healing rate of bone graft were evaluated and compared, and the factors affecting the healing of artificial bone were analyzed. Results The initial healing rate of hydroxyapatite was 62.9%, the initial healing rate of allogeneic bone was 96.8%, the difference was statistically significant (P = 0.000); the average initial healing time of hydroxyapatite was (13.3 ± 7.5) The mean initial healing time of allogeneic bone was (8.8 ± 4.9) months, the difference was statistically significant (P = 0.000). Gender (P = 0.369), age (P = 0.290), lesion site (P = 0.704), disease type (P = 0.994) and bone graft volume (P = 0.348) were not significant factors that affected hydroxyapatite healing . The MSTS scores of patients in hydroxyapatite group and allogeneic bone group were (28.8 ± 1.8) and (29.0 ± 1.5) points respectively, with no significant difference between the two groups (P = 0.450). Conclusions Hydroxyapatite was used to reconstruct bone defects after resection of benign bone tumors. The postoperative function was satisfactory, but the healing rate and healing rate were lower than that of allogeneic bone.
