目的探讨尺骨近端骨肿瘤的手术方式及疗效。方法回顾1例行定制型肘关节假体置换术治疗尺骨鹰嘴骨巨细胞瘤病例并进行肘关节功能评价;计算机检索1990~2014年,Pubmed、Cochrane图书馆、荷兰医学文摘(Embase)、万方数据库、中国期刊网全文数据库(CNKI)和中国生物医学文献数据库(CBM)发表的文献,手工检索已发表的骨科论文及查阅相关参考文献,对纳入文献进行研究并讨论。结果本例患者随访5个月,术后患者肘关节功能恢复满意,Mayo肘关节功能评分从术前50分升至术后5个月80分,肘关节屈伸范围从60°(30~90)°改善至120°(0~120)°。6篇文献详细报道了尺骨近端肿瘤的手术方式、疗效及并发症,其中4篇报道7例尺骨近端骨巨细胞瘤病例。结论尺骨近端骨巨细胞瘤患者行定制型肘关节假体置换治疗,术后随访效果满意;对尺骨近端恶性肿瘤、复发肿瘤及浸润范围过大肿瘤,定制型肘关节假体置换术近期效果满意。
Objective To explore the surgical methods and curative effects of proximal ulnar bone tumors. Methods A retrospective review of a routine custom-made elbow prosthesis replacement for the treatment of olecranon giant cell tumors and evaluation of elbow function; computer search 1990-2014, Pubmed, Cochrane Library, Dutch Medical Abstracts (Embase), 10,000 Documents published by the Chinese database, CNKI, and the Chinese Biomedical Literature Database (CBM) were manually searched for published orthopedic papers and referenced references to study and discuss the included literature. Results The patient was followed up for 5 months. The patient’s elbow function recovered satisfactorily. The Mayo’s elbow function score rose from 50 preoperatively to 80 minutes postoperatively. The elbow flexion ranged from 60° (30-90). ° Improved to 120° (0~120)°. Six papers reported in detail the surgical approach, efficacy, and complications of proximal ulnar tumors, of which four reported 7 cases of proximal ulnar giant bone cell tumors. Conclusion Custom-made prosthetic elbow prosthesis replacement for patients with giant cell tumor of proximal ulna is satisfactory. The follow-up results are satisfactory. For the proximal ulnar malignant tumors, recurrent tumors, and infiltrative tumors, custom-made elbow prosthesis replacement surgery has been performed recently. Satisfactory results.
