目的 探讨松果体实质肿瘤的组织学分型、分化及其发病和预后的特点。方法 应用HE染色对 2 1例松果体实质肿瘤进行组织学分型 ,对其中 2 0例行突触素、神经元特异性烯醇化酶(NSE)、神经丝蛋白 (NF)、神经母细胞瘤 (NB)、胶质纤维酸性蛋白 (GFAP)、细胞角蛋白 (CK)、结蛋白免疫组织化学SP法染色 ,并进行临床资料分析及随访。结果 2 1例松果体实质肿瘤标本中 ,松果体细胞瘤 9例 ,松果体母细胞瘤 4例 ,中间分化型松果体实质肿瘤 8例。免疫组织化学染色 :突触素、NSE2 0例阳性 ,NF 14例阳性 ,GFAP 15例阳性 ,NB 2例阳性 ;结蛋白和CK全部阴性。结论 松果体实质肿瘤中明显具有松果体细胞瘤和松果体母细胞瘤两个阶段特点以及具有中间分化、移行特征者应归于中间分化型松果体实质肿瘤 ;该肿瘤可向神经元分化 ,未发现向上皮及肌肉分化 ;术后 1年内死亡率较高
Objective To investigate the histological type and differentiation of pineal parenchymal tumor and the characteristics of its onset and prognosis. Methods Twenty-one cases of pineal parenchymal tumors were stained with hematoxylin-eosin (HE) stain. The synaptophysin, neuron specific enolase (NSE), neurofilament (NF), neuroblastoma (NB), glial fibrillary acidic protein (GFAP), cytokeratin (CK) and desmocollin immunohistochemical SP staining. The clinical data were analyzed and followed up. Results Among pineal parenchymal tumors, there were 9 cases of pineal cell tumor, 4 cases of pinealoblastoma and 8 cases of parenchymatous pineal parenchymal tumor. Immunohistochemical staining: Synaptophysin, NSE2 0 cases were positive, NF 14 cases were positive, GFAP 15 cases were positive, NB 2 cases were positive; Desmin and CK were all negative. Conclusions The pineal somatic cell tumor and pinealoblastoma obviously have the characteristics of two stages as well as the characteristics of intermediate differentiation and migration in the parenchymatous pineal tumors, which should be attributed to the intermediate differentiated pineal parenchymal tumor.
