目的了解1993-2014年江苏省昆山市糖尿病死亡率变化趋势及定量估计影响因素,为糖尿病预防控制工作提供科学依据。方法收集历年江苏省昆山市分性别和年龄组的糖尿病死亡病例,计算粗死亡率和年龄标化率(2000年第5次人口普查年龄结构,简称“中标率”)。使用年度变化百分比(APC)评价糖尿病死亡率在年份之间变化趋势;采用率差别分解分析人口因素对糖尿病死亡率变化的贡献。结果 22年间糖尿病粗死亡率在总人群(APC=5.3%,95%CI:4.2%~6.4%,P<0.01)、男性(APC=4.7%,95%CI:3.3%~6.1%,P<0.01)及女性(APC=5.8%,95%CI:4.4%~7.2%,P<0.01)中明显上升。糖尿病死亡率中标率在总人群(APC=2.3%,95%CI:1.2%~3.4%,P<0.01)、男性(APC=1.8%,95%CI:0.3%~3.3%,P<0.05)及女性(APC=2.6%,95%CI:1.2%~4.0%,P<0.01)中也明显上升。与1993年相比,2014年糖尿病死亡率在男女性合计、男性及女性人群分别上升了14.99/10万、13.25/10万和16.71/10万,这种上升主要是非人口因素所致,其贡献分别为61.34%、60.36%和61.57%。结论 1993-2014年江苏省昆山市糖尿病死亡率快速上升,其中非人口因素是主要的促进因素之一,探寻具体的非人口因素是当前糖尿病防控的重点。
Objective To understand the trend of diabetes mortality in Kunshan City, Jiangsu Province from 1993 to 2014 and the influential factors of quantitative estimation, and provide a scientific basis for the prevention and control of diabetes. Methods The deaths from diabetes mellitus and age group in Kunshan City of Jiangsu Province in previous years were collected, and the crude mortality rate and age-standardized rate (the fifth census age structure in 2000, referred to as “successful rate”) were calculated. The annual percentage change (APC) was used to evaluate the trend of diabetes mortality in different years. The difference of population demographics was used to analyze the contribution of population factors to the changes of diabetes mortality. Results The crude mortality rate of diabetes was significantly higher in the general population (APC = 5.3%, 95% CI: 4.2% -6.4%, P <0.01) 0.01) and female (APC = 5.8%, 95% CI: 4.4% -7.2%, P <0.01). The prevalence rate of diabetes mortality was in the general population (APC = 2.3%, 95% CI: 1.2% -3.4%, P <0.01) And women (APC = 2.6%, 95% CI: 1.2% -4.0%, P <0.01). Compared with 1993, the overall mortality rate of diabetes in 2014 was 14.99 / 100,000, 13.25 / 100,000 and 16.71 / 100,000 respectively for men and women. Conclusion The mortality rate of diabetes in Kunshan City, Jiangsu Province, increased rapidly from 1993 to 2014. Among them, the non-demographic factors were one of the major contributing factors. Exploring the specific non-demographic factors is the current focus of diabetes prevention and control.
